سونوگرافی حجم رحم 4سی سی و ابعاد ان

Definition:
• Onset of secondary sexual characters before the age
of 8 years in girls and 9 years in boys.
• In girls, the initial
development.
•
physical change is breast
In boys, the initial physical change is testicular
growth.
Classification:
Gonadotropin dependent (central).
Gonadotropin independent (peripheral).
Incomplete.(premature telarche,premature
pubarche ,premature menarche)
CPP:
Early activation of the HPG axis.
5- to 10-fold more often in girls than in boys .
Usually sporadic.
90% of girls have an idiopathic form.
Structural CNS abnormality in up to 75% of boys
.
CPP:cont
Affect child's growth potential.
Height, weight, and osseous maturation
are advanced.
Short ultimate stature.
Mental development is usually compatible with
chronological age.
Emotional behavior and mood swings are common.
Causes of CPP:
Idiopathic
Organic brain leision
hypothalamic hamartoma
brain tumors, hydrocephalus,severe head
trauma,myelomeningocele
Hypothroidism(prolonged and untreated).
Previous excess androgen exposure.
Radiation
Radiation:
• Low-dose radiation (18-24 Gy) hastens the onset of
puberty almost exclusively in girls.
• High-dose radiation (25-47 Gy) appears to trigger
precocious sexual development in both sexes.
• Hypopituitarism with gonadotropin deficiency can
subsequently develop as a late effect of high-dose CNS
irradiation.
Syndrome of Precocious Puberty
and Hypothyroidism:
• In children with untreated hypothyroidism, the onset of
puberty is usually delayed until epiphyseal maturation
reaches 12-13 yr of age.
• Precocious puberty in a child with untreated
hypothyroidism and a prepubertal bone age presents a
strikingly unphysiologic association.
• It is common and occurs in as many as 50%
of children with severe hypothyroidism of long duration.
Cont:
• These children have the usual manifestations of
hypothyroidism, including retardation of growth
and of osseous maturation .
• The cause of the hypothyroidism is usually
Hashimoto thyroiditis.
Cont:
• Sexual development in girls consists primarily of breast
Enlargement and menstrual bleeding; the latter can
occur even in girls with minimal breast enlargement.
• Pelvic sonography can reveal large, multicystic ovaries.
• Enlargement of the sella, which is typical of
long-standing primary hypothyroidism, may be
demonstrated by skull film or MRI.
Cont:
• TSH is markedly elevated, often >500 /lU/mL.
• PRL is mildly elevated.
• FSH is low and LH is undetectable but massively
elevated TSH interact with the
FSH receptor
(specificity spillover), thus inducing FSH-like effects in
the absence of LH effects on the gonads.
• The precocious puberty associated with hypothyroidism
behaves as an incomplete form of gonadotropindependent puberty.
Cont:
• Treatment of the hypothyroidism results in rapid
return to normal of the biochemical and clinical
manifestations.
• Rapid bone age advancement and possible
progression to central puberty could occur in the
months following the initiation of thyroid hormone
replacement, a complication that justifies delaying
puberty with GnRH analogs.

Patterns of CPP:
Rapidly progressive(Most girls particularly
<6 yr and a large majority of boys).
Slowly progressive (girls >6 yr).
Spontaneously regressive(small percentage of girls )
need for longitudinal observation at the onset of sexual
development, before treatment is considered
LABORATORY FINDINGS:
Increase Sex hormone concentrations.
Serum estradiol in girls is low or undetectable
in the early phase of sexual precocity.
serum LH is detectable in 50-75% of girls.
Lab:cont
GnRH stimulation test is a helpful diagnostic tool.
A brisk LH response (LH peak >5-10 lUlL) with
predominance of LH over FSH tends to occur early
in the course of precocious puberty.
Lab:cont
In girls nocturnal LH and LH response to GnRH
may be quite low at breast stages II to early
III.
In girls LH: FSH ratio can remain low until midpuberty.
Lab:Cont:
Osseous maturation advanced(more than 2-3SD).
Pelvic ultrasonography in girls reveals progressive
enlargement of the ovaries and uterus to pubertal size.
Upper limit of uterine length in prepubertal state is
3.5 cm , volume is 1.8 ml.
MRI scan demonstrates physiologic enlargement of
the pituitary gland and also reveal CNS pathology.
Indications for MRI:
Rapid breast development.
Estradiol >30 pglmL.
Girls <6 yr of age.
All boys
Some authorities recommend MRI scans for all children
with central precocious puberty.
Treatment of CPP:
• GnRH agonist.
• 60 to 120 micro/kg
• Gonadotropic cells require pulsatile, rather than
continuous, stimulation.
When stop treatment:
• We generally continue treatment until about age 11 in
girls, and age 12 in boys.
• The decision of when to discontinue GnRH agonist
therapy is individualized.
• Age of the child, bone age and height age, predicted
height, and social desire are important.
Adverse effects:
Except for a reversible decrease in bone density (of
uncertain clinical significance), no serious adverse
effects was seen.
Monitoring of bone density is not required .
We suggest ensuring adequate intake of
calcium and Vit D during treatment
Cont:
When therapy is discontinued, puberty resumes at a
"pubertal" chronological age.
In girls, menarche and ovulatory
cycles generally
appear at an average of 16 mo (range 6-24 mo) of
cessation of therapy.
long-term treatment with GnRH agonists does not appear to
cause or exacerbate obesity

Gonadotropin-independent
p.puberty:
Excess sex hormones ,which is independent of
the HPG axis.
It is isosexual or contrasexual.
May present with some or all of the physical changes
of puberty.
FSH and LH levels are suppressed.
GnRH agonists are ineffective.
Causes:
Causes of GnRH independent isosexual precocity In girls,
include:
Ovarian cysts :(most common cause) .
Ovarian tumors :Granulosa-cell tumors, Leydig cell
tumors and gonadoblastoma.
Peutz-Jeghers Syndrome.
Adrenal Adenomas.
Maccune Albright syndrome
Ovarian cysts in infancy and childhood
• If cysts are asymptomatc ,less than 4-5cm and
simple, observation and serial sonography every 2
months is suffcient.
• In functional ovarian cysts which causes psedo
percocious puberty treatment with medroxy
progestrone is needed.
• In case of recurrent ovarian cysts MAC should be
considerd.
MAC:
McCune-Albright syndrome :
It is a rare disorder defined as the triad of:
Peripheral precocious puberty
Café-au-lait skin pigmentation
Fibrous dysplasia of bone.
Cont:
• Caused by a missense mutation in the gene
encoding the a-subunit of Gs, that stimulates cAMP
formation.
• Activation of receptors ACTH, TSH, FSH, and LH
receptors that operate via a cAMP-dependent
mechanism.
Cont:
• Precocious puberty predominantly in girls about 3 yr.
• Suppressed levels of LH and FSH.
• No response to GnRH stimulation.
• Estradiol levels vary from normal to markedly elevated
(>900 pg/mL).
• Cyclic ovarian cysts.
Cont:
Hyperthyroidism
Cushing syndrome
Gigantism or acromegaly
Phosphaturia
Cardiovascular involvement
Hepatic involvement
Treatment:
Functioning ovarian cysts often disappear spontaneously.
In girls :
aromatase inhibitors such as letrozole (1.25-2.5 mg/day
orally) or
antiestrogens such as tamoxifen; fulvestrant .
In boys:
The same components in combination with
Antiandrogens (such as spironolactone 50-100 mg bid), or
flutamide( 125-250 mg bid).
Cont:
• Long-acting analogs of GnRH is indicated only
for patients whose puberty has shifted from a
gonadotropin- independent to predominantly
gonadotropin-dependent mechanism

کوچک است(کمتر از 5سانتیمتر)نیاز به درمان ندارد.
باید در فواصل 2ماهه بیمار رابا سونوگرافی پیگیری کرد •
در صورت افزایش سایز یا ایجاد عالمت درمان مدروکسی •

Incomplete precocity:
• Premature thelarche
• Premature adrenarche
• Premature menarche
Incomplete precocity:
• It is a variant of normal puberty.
• Growth is not accelerated.
• Close monitoring is needed, as a significant number
of these children may develop central precocious
puberty.

Premature telarche:
Transient condition of isolated breast development
often appears in the first 2 yr of life.
It can be present at birth and persists.
It may be unilateral or asymmetric and often
fluctuates in degree.
Cont:
Growth and osseous maturation are
normal or slightly advanced.
The genitalia show no evidence of estrogenic
stimulation.
Cont:
• It is usually sporadic.
• Breast development might regress after 2 yr,
often persists for 3-5 yr, and rarely
progressive.
• Menarche occurs at the expected age, and
reproduction is normal
Lab:
• Basal serum levels of FSH and the FSH response to
GnRH stimulation may be greater than that seen in
normal girls.
• Plasma levels of LH and estradiol are consistently
less than the limits of detection.
• Ultrasound examination of the ovaries reveals
normal size, but a few small «9 mm cysts are not
uncommon.
Exaggerated or atypical thelarche:
• In some girls, breast development is associated with
definite evidence of systemic estrogen effects, such as
growth acceleration or bone age advancement.
• Pelvic sonography might reveal enlarged ovaries
or uterus.
Cont:
• It differs
regress.
from CPP and spontaneously
• Leuprolide or GnRH stimulation elicits a robust
FSH response, a low LH response, and (after
leuprolide only) a moderate estradiol
increment at 24 hr (average 60-90 pg/mL).
Pathogenesis:
• The pathogenesis
is
unclear.
• Activating mutations of the GNASl gene encoding the
a-subunit of the Gs protein have been described in
some patients without other signs of McCuneAlbright syndrome.
Cont:
• FSH , LH, and estradiol are generally low .
• Pelvic ultrasound examination is rarely
indicated.
• Bone age should be obtained
• Regression and recurrence

than benign premature thelarche.
Treatment:
• It is a benign condition .

Premature adrenarche:
• Appearance of pubic and/or axillary hair prior to the
age of 8y in girls or age 9 years in boys.
• No other signs of puberty or virilization.
• It is about 10 times more common in girls than boys.
• The prevalence is increased in children with CNS
abnormalities.
Cont:
• Serum DHEAS concentration is appropriate for
pubic hair stage.
• serum LH, FSH and gonadal steroid are in the
prepubertal range.
• 20 percent of girls with premature adrenarche have
been reported to progress to central precocious
puberty.
Cont:
• Premature adrenarche is a slowly progressive
condition that requires no therapy.
• A subset of patients have one or more features of
systemic androgen effect, such as marked growth
acceleration, clitoral (girls) or phallic
(boys)
enlargement, cystic acne, or advanced bone age (>2
SD above the mean for age).
Cont:
• In these patients an ACTH stimulation test with
measurement of steroid intermediates (mainly 17-OH
p) is indicated to rule out nonclassical CAH due
to 21-hydroxylase deficiency.
• Follow up of all children with premature
adrenarche is strongly recommended.
Cont:
• It is a risk factor for PCO.
• Progression to pubertal hyperandrogenism may
be prevented by insulin- sensitizing agents
(metformin 850-1000 mg/day).

Premature Isolated Menarche :
• Periodic vaginal bleeding at age 1 to 9 years without
any other signs of secondary sexual development.
• Causes :
McCune-Albright syndrome
Hypothyroidism
Exposure to exogenous estrogens
Neoplasms, granulomas, infection
Foreign body
Trauma (sexual abuse)
Urethral prolapse
Cont:
• Etiology is uncertain.
• There is a predominance of FSH secretion.
• At the normal age of puberty, secondary
sexual development and menses ensue and
follow a normal pattern, as does stature
،
