고관절의 병변
1. 일과성 고관절 활액막염(transient synovitis of hip)
: 소아기에 발생하는 고관절의 비특이성 염증 질환으로
특별한 치료 없이도 후유증 없이 저절로 치유; observation hip

severity of hip irritation
and aid in distinguishing
between transient synovitis
of the hip and septic
arthritis. The diagnosis of
transient synovitis is more
likely if an arc of 30
degrees or more of hip
rotation is possible without
pain.
2. 고관절 주위의 점액낭염
1) Trochanteric bursitis
2) Ischiogluteal bursitis = tailor or weaver’s bottom
Ischiogluteal bursitis is inflammation
of the bursa that lies between the
ischial tuberosity and the tendon of a
hamstring muscle
3) Iliopectineal bursitis
L1 = iliopubic or iliopectineal (arcuate) line
L2 = anterior rim of the acetabulum
L3 = ilioischial line, formed by the posterior
portion of the quadrilateral plate (surface)
of the iliac bone
L4 = posterior rim of the acetabulum
L5 = teardrop, formed by the medial
acetabular wall, the acetabular notch, and
the anterior portion of the quadrilateral
plate

4. 대퇴 골두 무혈성 괴사
(avascular necrosis of femoral head)
• 대퇴골 두가 순환 장애로 인하여 괴사에 빠짐으로 해서
일어나는 질환
원인 : 고관절 부위의 외상, 부신피질호르몬 투여, 과다한
음주, 잠수병, 겸상적혈구증, 방사선 조사, gaucher병, 통풍,
정맥혈전증, 혈청지질 이상, 전신성 홍반성 낭창, 만성 신질
환, 장기이식, 흡연

: 편평 고라고도 하며 소아에 발생하는 자기한정성
고관절 질환으로 대퇴골 두의 골핵에 다양한 정도
의 괴사가 일어나는 질환
6. 대퇴 골두 골단 분리
(slipped capital femoral epiphysis)
특별한 외상없이 대퇴골 두의 골단이 골단판을 통해 골간단
부로부터 분리되어 대퇴골 간부가 전회측으로 회전 이동을
하고 점차 상방으로 이동하며 대퇴골 두는 비구 내에서 후
내방으로 전위되는 질환
7. 발음성 고관절(snapping hip)
슬부 병변
1.슬관절 주위의 점액낭염
1) 점액낭염(bursitis)
2. 슬내장
(internal derangement of knee)
• 일반적으로 외상 후, 슬관절에 운동통, 관절액
증가, 운동 제한 등의 기능 장애가 일어나는 상
태를 총칭하여 슬내장이라 한다.
• 외상이 원인일 때에는 그 기전이 뚜렷하여 손상
된 부위의 진단이 용이하나 그 기전이 애매하여
슬관절의 어느 조직이 손상되었는가를 확진하기
어려울 경우가 많다.
• 슬내장의 빈번한 원인 : 반월상 연골, 측부 인대,
십자인대, 경골 극의 손상, 관절 내 유리체, 슬개
하 지방체 비후, 활막추벽 증후군 경골 주위의
외골종
1) 반월상 연골 손상(injury of meniscus or
semilunar cartilage of knee)
• Radial tear.
Vertical longitudinal (bucket-handle) tear
Horizontal cleavage tear.
Radial tear.
Complex degenerative tear.
2) 측부 인대 손상
(injury of collateral ligament)

3) 십자인대 손상
(injury of cruciate ligament)
가. 전방십자인대손상
• Assessment of anterior
cruciate ligament (ACL)
stability. The anterior
drawer test (top) is
performed with the knee in
90 degrees of flexion.
Similar to the Lachman's
test (bottom), where the
knee is in 20 degrees of
flexion, the tibia is drawn
anteriorly, and asymmetric
translation is an indicator
of ACL injury.

5. 슬개골의 병변
1) 슬개골 재발성 탈구(recurrent dislocation of patella)
2) 슬개골 연골 연화증
(chondromalacia patellae)
3) 이분 슬개골
(bipartite patella)
• 골화 중심들의 유합부전이 발생하여 2분열, 3분열 혹은
다분열의 슬개골 형성
6. Osgood-schlatter
osteochondritis of tibial tuberosity,
partial separation of tibial tuberosity
7. Snapping knee
• 슬관절부 마찰음
• 동통은 호소하지 않음
• 대부분 외측 반월반 연골의 전위 또는 원
판형 연골을 가지고 있는 예에서 나타남
• 대퇴 이두건, 반건양건 또는 장경인대에
의한 급격한 방향 전환시에도 나타남
