AFFIRM Analyse während Behandlung: SR ist mit Überlebensrate korreliert
Implikationen
Bei AFib-Patienten wie den Teilnehmern an der AFFIRM-Studie verbessert Warfarin die Überlebensrate. Vorhandensein des SR, nicht aber Verwendung von Antiarrhythmika ist mit einer geringeren Mortalität verbunden.
Diese Ergebnisse legen nahe, dass, wenn es eine wirksame Methode zur Aufrechterhaltung des SR mit weniger Nebenwirkungen gäbe, diese die Überlebensrate verbessern könnte

ATHENA Studie: Einschlußkriterien
≥75 yrs with or without additional risk factors ≥70 yrs with at least one of the following risk factors: arterial hypertension (ongoing therapy with at least two antihypertensive drugs of different classes), diabetes mellitus, prior stroke or transient ischemic attack or systemic embolism, left atrium diameter ≥ 50 mm by M mode echocardiography, LVEF < 0.40 by 2D echocardiography.

Compared to placebo, Multaq ® (dronedarone) significantly decreased the risk of cardiovascular death by 30% (p=0.034).
Multaq ® (dronedarone) was associated with numerically fewer deaths from any cause (16%, p=0.17).
First cardiovascular hospitalization was reduced by 25% (p=<0.001).

Death from arrhythmias was reduced by 45% (p=0.01) when patients were treated with Multaq ® (dronedarone).
Multaq ® (dronedarone) demonstrated a lower risk of pro arrhythmia than placebo and no excess of hospitalizations for congestive heart failure.
The rate of study drug discontinuation was similar between the two study arms.

Rote-Hand-Brief Es sind Fälle von Leberschädigung bei Patienten, die mit Dronedaron behandelt wurden, berichtet worden. In zwei Fällen wurde aufgrund eines akuten Leberversagens eine Lebertransplantation erforderlich. Einige dieser Fälle traten kurz nach Behandlungsbeginn auf.
Bei Patienten, die mit Dronedaron behandelt werden, sollten Leberfunktionstests durchgeführt werden, und zwar: vor Behandlungsbeginn und anschließend monatlich für einen Zeitraum von sechs Monaten sowie im Monat 9 und 12 der Behandlung, danach in periodischen Abständen.
Patienten, die zurzeit mit Dronedaron behandelt werden, sollten innerhalb des nächsten Monats kontaktiert werden, um Leberfunktions-tests durchzuführen. Weitere Kontrollen sollten entsprechend der oben aufgeführten Empfehlungen in Abhängigkeit vom Behandlungsbeginn durchgeführt werden.
