شرح حال بيمار :
زن 48ساله ای با سابقه دو ساله عالئم یبوست ،دل درد و دل پیچه
اطراف ناف و زیر دل ،نفخ مراجعه کرده است در بررسی های
پاراکلینیک کم خونی ،کم کاری تیروئید ،اختالالت کلسیم و فسفر
وجودندارد ،بیمار اظهار می دارد که در این مدت با تشخیص کولیت
تحت درمان با سولفاساالرین ،کلیندیوم ،هیدروکسید منیزیم و
پسیلیوم بوده است و تاحدودی احساس بهبودی میکند .بیمار اظهار
می دارد که هرماه یک باردچار عالئم سردرد ضرباندار با احساس
تهوع خفیف می شود که یک روز ادامه می یابد و در بررسی نوار
مغز و سی تی اسکن مغزی موردی نداشته است
و با تشخیص میگرن تحت درمان با سدیم والپرات و قرص
پروپرانالول است ولی هنوز از سردرد پیشنانی و فرق سر
بخصوص در عصرها شاکی است .قاعدگی بیمار نامنظم
است ولی دردهای شکمی و نفخ وی از روز قبل از قاعدگی
تا سه روز اول قاعدگی شدید تر می شود و برای تسکین آن
کپسول مفنامیک استفاده می کند .بیمار اظهار می دارد که
مکرراً دچار درد و گرفتگی در پشت شانه سمت راست می
شود که برای تسکین آن گهگاهی از قرص متوکاربامول
استفاده می کند .
تقریبا هفته ای دوبار احساس داغ شدن در سروسینه دارد که
متعاقب آن آب بیشتری می نوشد ولی با افزایش مصرف آب
هر ربع ساعت احساس نیاز به دفع ادرار پیدا می کند در
آزمایش ادرار وی یک بار 3-2گلبول سفید در بررسی
میکروسکپی با درشت نمایی باال گزارش شده است ولی
بیمار اظهار می دارد با پنج روز خوردن سیپروفلوکسامین
عالئم درد شکمی وی تشدید شده است .باتوجه به احساس
درد شدید شکم در ناحیه زیر دنده ها بخصوص در یک
نقطه در سمت چپ دو هفته امپرازول خورده است
ولی بهبودی واضحی در این ناحیه احساس نمی کند ،بیمار از
پنج سال پیش احساس درد در ناحیه چپ سینه داشته است
که چند بار به اورژانس مراجعه می کندو در نوار قلب ،
تست ورزش و اسکن تالیوم قلب مساله ای ندارد .در اکو
کاردیوگرافی پروالپس میترال درجه پایین گزارش می شود
و به وی توصیه می شود پروپرانالول را ادامه دهد ولی
باتوجه به تنگی نفس وخس خس سینه که در اسپیرومتری
عالئم واضحی نداشته است به وی توصیه شده است باتوجه
به
احتمال آسم پروپرانالول را کمتر بخورد و برای یک ماه
اسپری بکلومتازون و سالمترول استفاده کند .بیمار هر روز
می کند و از درد سردل بعد غذا شاکی است ولی
ترش
اندوسکپی وی طبیعی بوده است .عالئم ترش کردن و درد تا
حدود 80درصد با امپرازول بهبودی می یابد ولی با قطع
دارو عود می کند .
