death worldwide)

• It is estimated that 1/3 of all cancers are preventable,
1/3 are curable with early diagnosis.
• In the rest 1/3 we can increase the quality of life with
conservative & alleviative treatment
• In spite of increase in public awareness about cancer
control still there is a lack of national commitment

نتیجه تغییرهای مشابه در ژنها)
دومین علت مرگ و میر در آمریکا و بعضی کشورهای
اروپایی
در کشورهای در حال پیشرفت رو به افزایش است.

• In epidemiology precancerous lesions are very
important in cancer prevention:
– A key for assessing mechanism of cancer genesis
– Abnormal proteins can be used in searching cancer and
diagnosing individual in risk.
– Tracking the course of precancerous lesions
identifying the etiology of cancer and causes of
aggravation
– Prevention of more invasive and fatal form of cancers
– The incubation period of some cancers are very long
Etiology of cancer
• Most is not identified
• First time: relationship of some occupations with cancer
– Scrotal cancer in smoke pipe cleaners
– Bladder cancer in chemical workers (aniline)
– 2-6% of all cancers are attributable to occupational contacts
• Estimation of cancer mortality (wide range=unreliability in
determining etiology):

– Immunosuppressant: azathioprine, cyclopurine A

– Benzopyrene, dibenzoanthrasen, nickel, cadmium,
formaldehyde
– Nicotine

– Lung(11-22 ×, dose related), pharynx, mouth, larynx,
esophagus, prostate, kidney, pancreas, bladder, cervix
– 25-40% of all cancer deaths are attributable to tobacco
– Passive smoking: lung
– Pipe or cigar= larynx, pharynx, esophagus
– Uranium, asbestos increase mortality of lung cancer
– 1960-80: lung cancer mortality rates increased 76% in
male & 135% in female( USA &Canada>300%,
Australia, Denmark & News land>200%,Japane,
Norway, Poland, Sweden & UK>100%
– WHO: 33% of 15 y/o males in developed countries
&50% of 15 y/o in developing countries are smoker.
– The rate of smoking increase in females are more
rapid than males.
– In USA: MR rate of lung cancer is decreasing in elderly
males.
• 3-Alcohol
– Interaction with tobacco: mouth & esophagus
– Synergism with tobacco: mouth, pharynx, larynx, esophagus( not lung)
– Liver, Breast?

Esophagus: fried &salted, Alcohol (vegetables)
Liver: nitrosamines
Colorectal & Breast: Fried (vegetables, Ca , Fruits)
Endometrial & Ovary: Hydroxyl Radicals( vegetables, Fruits)
Fiber: transmission rate of GI content, concentration of carcinogens,
transition time
– Food preparation: smoked, grilled, repeated Frying in oil (benzopyrene)
– High heat in caramelization of sugar: mutagenic
factors( not in ordinary cooking)
– Preservative, additives, essence, synthetic
sweeteners
– Saccharine: carcinogen in high amount in rat(
bladder)
no importance in usual amount in human, no
epidemiologic evidence on increasing risk of cancer
in human
– Nitrates or nitrites( as preservative to meat): only
10% as preservative but may be important because
it can reach GI in high concentration
• 5-Biologic factors
–15% of all cancers worldwide due to
infectious agents
–HIV, HBV, HCV, HPV, H.pylori
–Shistosomiasis
• 6-Genetic & environmental factors
–Retinoblastoma in a family, Leukemia in
Mongolian children,
• But Genetic factors do not have an important role in initiating
most of human cancers
• They are not obvious and recognizing them is difficult
• The origin of cancer is environmental factors than genetic
factors.
• Smoking, in gastric cancer mortality in 6 geographic region(
except East of Asia & Europe) due to more freezing and less salt
food.
• Mortality rate of breast cancer is growing worldwide & in all
age groups.( except Scandinavian countries & USA” the young”,
& Oceana”75-84 y/o” ).The least MR is in the East of Asia(
although growing)
Descriptive Epidemiology
• Age: an important factor
– Less in children &adolescence
– A disease of elderly
– Peak age is different in different cancers.
• Breast & uterus:4,5
• Prostate:>65
• Hereditary: early childhood
– Most cancers have long incubation period.
• Sex
– Frequency of most cancers in both sexes is similar
– Male; prostate, skin
– Female: Breast, uterus, cervix
• Occupation
–Contact with dangerous chemical,
biological and physical material
direct relationship with cancer
–Developing countries: great problem with
occupational hazards( rapid technology,
smoking). The rate of cancer is rapidly
growing.
• Risk Factors
– RR>4: highly carcinogen (smoking &occupational
factors, lung cancer)
– 2-4:medium(Radon, lung cancer)
– <2:weak(fat & fiber, colorectal cancer)
– Geographic distribution
• More prevalent in developed countries( growing from
1930)
• Cancer specific MR in 1996:6 million(3 million for
lung(989000), stomach, intestine, liver & breast)
• MR in 1996: lung> stomach> intestine> liver> breast
– Different part of the world, Different incidence rate
of malignant neoplasm
• China, Kenya, Mozambique: HCC due to aphlatoxin
• India: oral cavity carcinoma because of chewing beten

• Egypt: bladder carcinoma due to schistosomiasis
• Japan: gastric cancer (rare cases of MM, Hodgkin
lymphoma, prostate, uterus, ovary & breast)
• North-East of Iran, Central Asia, Northern China:
Esophageal cancer
• China, South- East Asia: nasopharynx
Time Trend
• Incidence in USA:
– 1930:143
– 1950:157
– 1990:174
– 2007: 156
• The most important cause of increase is due
to lung cancer (smoking)
Causes of increase in cancer incidence
• Long incubation period: cancer may be the result
of a contact in some decades ago.
• Mean age of population
– Mean age at diagnosis:67 y/o
• Decrease in other causes of mortality (infectious
diseases and CVD): more people live a long life,
so more risk of cancer
• 18% of men & 16% of US women born in 1950 & 25% of
men & 20% of women born in 1975 will die from cancer.
• 33% of people living in developed countries will get a
type of cancer.
• In developing countries the figures are lower but
incidence & mortality increasing.
• 1930-1990: mortality of
– Breast cancer: no change
– Uterus: decreased
– Lung: increased
• From 1980: increase in BRC incidence (some due to
mammography)

اگر چه میزانهای کلی مرگ و میر از سرطان از دهه 1950به طور قابل مالحظه ای
تغییر نکر ده است ,میزان درمان پذیری لوکمی کودکان ,لنفوم بورکیت و بیماری
هوچکین ,سرطانهای بیضه و کوریو کارسینوم به طور قابل مالحظه ای بهبود یافته
است.اما چون این تومورها در مقایسه با سه سرطان شایع(ریه ,پستان
,کولورکتال که به تنهایی باعث % 50تمام موارد مرگ و میر ناش ی از سرطان
هستند)سرطانهای نادری هستند,لذا موفقیتهای درمانی آنها تاثیری در مرگ و میر
کلی ناش ی از انواع سرطان نداشته است.
افزایش در مرگ و میر در حد اپیدمی :ریه و مالنوم
< 65سال:مرگ و میر اکثر سرطانها کاهش یافته است.
اپیدمی سرطان ریه در زنان:مرگ و میر ریه و پستان در زنان آمریکا در 1985مساوی
ودر 1986ریه بیشتر از پستان بوده است.
البته هنوز مجموع مرگ و میر ناش ی از سرطان ریه هنوز در مردان بسیار باالتر
است اگر چه سرعت افزایش در زنان در چند سال گذشته باالتر بوده است.

سایر سرطانهایی که مرگ و میر آنها در حال افزایش است:
مغز و لوکمی و پانکراس(>) 65و ریه (مردان > )65و
مالنوم ولنفوم غیر هوچکینی
شاخص تشخیص و درمان خوب سرطان در طول زمان و
شیوه دیگر اطالع از روندهای مرگ و میر ناشی از سرطان
:میزان بقای 5ساله
افزایش میزان بقای 5ساله :مالنوم ,تومورهای بیضه,مثانه,
هوچکین ,لوکمی و سرطانهای کودکان
میزان بقای 5ساله کمتر از :% 20معده ,ریه ,پانکراس ,کبد
و مری
تفاوتهای مرگ و میر و میزان بقای نسبی بین دو
جنس و نژادهای مختلف
• مردان سیاهپوست>مردان سفید پوست>زنان سیاهپوست و
سفید پوست
• علل:تفاوت در شیوه زندگی(رژیم غذایی,دخانیات,تماس با
مواد سرطانزا
• بقای کمتر در اقلیتها:تشخیص دیرتر به علت دسترسی کمتر
به خدمات بهداشتی و درمانی
اپید میو لوژی سرطان در ایران
• آمار موجود در مورد بیماری سرطان 3:مرحله زمانی
– قبل از تصویب قانون ثبت و گزارش اجباری موارد سرطان توسط
مجلس()1363
– بعد از تصویب قانون تا سال 1375
– بعد از (1375ویژگی این دوره تصویب و آغاز فعالیت طرح ملی
ثبت موارد سرطان توسط وزارت بهداشت است)
• : 1946-56بروز سرطان در ایران بسیار کمتر از
کشورهای همجوار بوده است(تخمین تنها بر اساس موارد
تایید شده آزمایشگاههای بافت شناسی)
– جنوب و غرب 3/28در صد هزار نفر(مردان 2/32و زنان
)2/24
– شمال 42در صد هزار نفر(مردان 50و زنان )3/34
– تخمین کل کشور به طور متوسط 50در صد هزار نفر
• 1363-67بیشترین فراوانی :پوست مری غدد لنفاوی پستان مثانه
• هیچکدام از آمارها مبتنی بر جمعیت نیستند.اما نکات مهم در تفسیر آمار
موجود:
– رشد سریع جمعیت طی دو دهه اخیر و تغییرات عمده در الگوی زندگی و
افزایش مصرف سیگار:
• افزایش پستان و کو لو رکتال و افزایش نسبی ریه.اما پوست فوقانی دستگاه
گوارش خون و غدد لنفاوی و بقیه مانند دوره قبلی بروز می نماید.
• افزایش نسبی سرطانهای کودکان به دلیل فراوانی نسبی جمعیت این گروه
سنی است.
– حد اقل تخمین های انجا م شده :هر سال حد اقل 80-120در صد هزار مورد
جدید انواع سرطان در کشور بروز می نماید و در سالهای آینده رو به تزاید
است.
• الزمه قطعی دسترسی به آمار معتبر و همه جانبه تحول از ثبت بر
اساس بیمارستان به ثبت بر اساس جمعیت است.
مرگ ومیر از سرطان در ایران
• آمار وزارت بهداشت :1375
– 185576مورد مرگ در سراسر کشور ثبت و گزارش شده است.
• گروه گردش خون %4/34
• سوانح و مسمومیتها و خود کشی %4/13
• سرطان و سایر تو مورها %4/9
– با توجه به سر شماری 60,055,488= 1375میزان مرگ و
میر از انواع ضایعه های نئو پالستیک > 29در صد هزار نفر
بوده است.یعنی 3برابر تفاوت بین میزان بروز و مرگ و میر
– بیشترین مرگ و میر ناشی از سرطان در اراک>آذر بایجان
غربی>کرمانشاه>گیالن

• Overall age-standard incidence rate per 100 000
population according to primary site is 110.43 in males and
98.23 in females.
• The five most common cancers (except skin cancer) are
stomach, esophagus, colon-rectum, bladder and leukemia
in males, and in females are breast, esophagus, stomach,
colon-rectum and cervix uteri.

• The estimated mortality rate for cancer was 41.1
and 65 per 100 000 for females and males in
2004.
• The current low cancer incidence rates in Iran
might be due to lack of national cancer screening
programs for prostate, colorectal or breast cancer,
a consequence of incomplete registration as well
as incomplete diagnosis of cancer patients; it is
expected that it will rise dramatically in the future
because of anticipated increase in life expectancy
and westernized lifestyle.
• سرطان پستان شایعترین و اولین سرطان در این کشور است.
• سرطان معده شایعترین سرطان در مردان ایران است و پس از آن سرطانهای مثانه،
پروستات ،روده بزرگ ،دستگاه خونساز ،ریه ،مری ،اعصاب مرکزی ،غدد لنفاوی و
پوست ،ده سرطان شایع مردان در ایران است”.
• شایعترین سرطانها در میان زنان شامل پستان ،پوست ،روده ،راست روده ،معده،
مری ،دستگاه خونساز ،تیروئید ،تخمدان و رحم
• سرطان معده در مردان و سرطان سینه در زنان شایعترین سرطانهای کشور ایران
هستند.

• Here in Iran, cancers are the third cause of
death with 30000 cases annually.

Cancer registry and comprehensive cancer control program (CCCP) in Iran’s national report
• cancer registry in Iran is started from 1363, when
registry and reporting of cancer cases was
approved by the parliament law and developed to
nowadays which 81% of whole cases are
registered entire country based on laboratory
diagnoses.
• completion of this registry is begin in four
provinces with adding hospital cases and death
certificates. Now cancer incidence in Iran is
70000 new cases annually (1/1000 inhabitants)
and
• age specific rate is 117.27 and 102.23 per
100000 for male and female respectively.

بیماران
• > %54مبتالیان با درمانهای موثر و همه جانبه مداوا می
شوند.در بقیه روند با یا بدون درمان ادامه می یابد.
• اثار روانی و عاطفی(بیماری تشخیص و درمان) روابط
خانوادگی زناشویی و اجتماعی درد و مسایل مالی و
اقتصادی بازگشت به کار
• مشکالت تغذیه ای عوارض
پیشگیری و کنترل
برآورد :حد اقل 1/3همه سرطانها قابل پیشگیری است.
