การตรวจครรภ์ : วัดระดับยอดมดลูก ( HF )

AC (Abdominal circumference) : เป็ นค่าที่ใช้ บอกการ
เจริญเติบโตของทารกได้ ดีที่สดุ โดยถือค่าที่ผิดปกติคือ น้ อยกว่า -2SD ที่อายุ
ครรภ์นนๆ
ั ้ โดยอัตราการโตของ AC ที่น้อยกว่า 1cm/2weeks บ่งชี ้ถึง
ภาวะ IUGR โดยมี sensitivity ถึง 84-100%
HC/AC ratio : ในทารกที่เป็ น asymmetrical IUGR จะพบ
ค่า HC/AC ที่มากขึ ้น โดยถือค่าผิดปกติที่มากกว่า +2SD ณ อายุครรภ์
นันๆ
้ อย่างไรก็ตามอัตราส่วนนี ้มีข้อจากัดในการทานายภาวะ
symmetrical IUGR เนื่องจากมีภาวะที่โตช้ าทุกๆส่วน

ให้ สงสัยภาวะ IUGR อย่างไรก็ตามการใช้ คา่ นี ้ในการวินิจฉัย IUGR มี
ค่าความไวที่ต่า
Estimated fetal weight : โดยการวัด BPD, HC, AC
และ FL โดยอาจเกิดค่าความคลาดเคลื่อนได้ ประมาณ 20% หากวัด EFW ได้
น้ อยกว่า 10 เปอร์ เซ็นไทล์ให้ สงสัยภาวะ IUGR
การวัดปริมาณนา้ คร่ า (Amniotic fluid volume) :
◦ ภาวะ IUGR มักพบเรื่ อง oligohydramnios ร่วมด้ วย
DVP < 2 cm หรื อ AFI < 5 cm
◦ หากในทารกรายใดที่มี IUGR ร่วมกับภาวะน ้าคร่ ามากผิดปกติ อาจสงสัยในเรื่ องโครโมโซมที่
ผิดปกติร่วมด้ วย
Placental grading : การตรวจพบ placenta grade 3
ในช่วงแรกของ 3rd trimester มักสัมพันธ์กบั IUGR

อัตราการเติบโตของทารกช้ า ซึง่ ประเมินได้ ด้วยการตรวจติดตามซ ้าใน 2-4 สัปดาห์

ที่มีความเสี่ยง
◦ ลด/งดการสูบบุหรี่
◦ การให้ ASA ขนาดต่าในสตีที่มีความเสี่ยงสูงตังแต่
้ อายุครรภ์น้อย เช่น HT, หรื อ มีประวัติเคย
คลอดบุตร IUGR มาก่อน ( RCOG แนะนาให้ ASA ในสตรี ที่มีความเสี่ยงสูงต่อ
preeclampsia เพื่อป้องกันการเกิด IUGR ได้ โดยควรเริ่มให้ ก่อนอายุครรภ์ 16 สัปดาห์ )
◦ รักษาปั จจัยเสี่ยงที่สามารถรักษาได้ เช่น malnutrition, anemia
แนะนาให้ พกั มากขึ ้น : นอนตะแคงซ้ าย
การเพิ่มสารอาหาร
การตรวจติดตามด้ วย ultrasound : ดู growth curve, AFI โดยควรตรวจ
ติดตามทุก 3-4 สัปดาห์
ตรวจติดตามสุขภาพทารกในครรภ์ : NST, BPP สัปดาห์ละ 1-2 ครัง้
Timing of delivery ขึ ้นอยูก่ บั สาเหตุที่เกี่ยวข้ อง รวมไปถึงผลการประเมิน
antenatal fetal surveillance
◦ คลอดเมื่อ GA 38-39+6 สัปดาห์ ใน isolated fetal growth restriction
◦ คลอดเมื่อ GA 34-37+6 สัปดาห์ ในรายที่มีความเสี่ยงอื่นๆร่วมด้ วย เช่น
oligohydramnios, maternal risk factor or comorbidity
