‫‪Dermatomycosis‬‬
‫سعفة األيدي ‪Tinea manuum‬‬
‫المسبب‪ -:‬فطر ‪ T.rubrum‬أو ‪ M.gypseum‬أو ‪T.mentagrophyte‬‬
‫هذا االسم يطلق على العدوى الفطرية بالفطريات الجلدىة التي تصيب اليد الواحدة أو‬
‫االثنتين معا ‪ .‬وهى أقل شيوعا وانتشا ار من سعفة القدم ( الفطريات التي تصيب القدم‬
‫صعب تشخيصها كثي ار ألنها تتشابه مع أعراض أمراض أخرى مثل ‪:‬‬
‫) وقد َي ُ‬
‫التهاب جلد اليد ‪ -‬وخاصة النوع الذي يسمى فقاع اليد والقدم ‪، Pompholyx‬‬
‫وهي عبارة عن ظهور فقاقيع في األيدي واألقدام ‪.‬‬
‫الصدفية ‪ -‬وخاصة ُبثار بواطن اليد والقدم ‪.palmoplantar pustulosis‬‬
‫القرنى التقشرى ‪.keratolysis exfoliativa‬‬
‫االنحالل َ‬
‫‪Dermatomycosis‬‬
‫السمات اإلكلينيكية ‪:‬‬
‫سعفة اليد يمكن أن تظهرعلى شكل طفح جلدي ملتهب حاد مثل سعفة الجسم ‪ ,‬وعادة‬
‫يكون للطفح حدود بارزة و يصفو فى وسطه ( سعفة القوباء الحَلقية )‪ .‬وهذا أكثر‬
‫شيوعا عندما تكون الفطريات المسببة للمرض هي فطريات أليفة الحيوانات (‬
‫‪ ) Zoophilic‬أوأليفة التربة (‪ )Geophilic‬وأمثلتها ‪:‬‬
‫وية إيرينسي (‪ )Trichophyton erinacei‬وتنتقل من القنفذ ‪.‬‬

‫‪Dermatomycosis‬‬
‫فى اكثراألحيان تسبب سعفة اليد مساحات رقعية من التقشير ‪ ,‬الجفاف والحكة‬
‫البسيطة على راحة اليد الواحدة ( سعفة فرط التقرن ) تتوسع تدريجيا ؛ وقد تسبب‬
‫زيادة وضوح عالمات الجلد ‪ .‬وعامة تنتشر إلى القدمين معا وفي هذه الحالة تسمى‬
‫ف الواحد" ‪ .‬والسبب المعتاد هو فطر أليف البشر ‪:‬‬

‫قد تسبب أيضا هذه الفطريات طفح فقاعى صغير متفرق على حواف وأطراف األصابع‬
‫ّ‬
‫أو راحة اليد ‪ .‬وتكون هذه الحويصالت الفقاعية على شكل مجموعات عنقودية تحتوي‬
‫على سائل صافى لزج ‪ ,‬وقد يكون لها طرف قشري ‪ .‬وهذا النوع من سعفة اليد يكون‬
‫بالحرقة‪.‬‬
‫مصحوبا بحكة واحساس َ‬
‫‪Dermatomycosis‬‬
‫العوامل المساعدة ‪:‬‬
‫تنتج سعفة اليد عن ‪:‬‬
‫االحتكاك المباشر بمكان عدوى آخر خاصة عدوى األقدام وعدوى األرفاغ‪.‬‬
‫االحتكاك المباشر مع شخص مصاب بالسعفة ‪.‬‬
‫المعدية ‪.‬‬
‫االحتكاك المباشر مع الحيوانات واألتربة ُ‬
‫استخدام ومالمسة أدوات ملوثة لشخص مصاب ( مناشف أو أدوات الحدائق) ‪.‬‬
‫ونجد هذا النوع من األمراض الفطرية بكثرة في األفراد الذين يعملون في األعمال‬
‫اليدوية ويتعرقون بغزارة في أيديهم ( مرض فرط التعرق ) وهؤالء المصابين بالتهاب‬
‫جلد اليد ‪.‬‬

‫األعراض المميزة لسعفة اليد ‪:‬‬
‫يمكن تمييز سعفة اليد من التهاب جلد اليد ‪:‬‬
‫في معظم حاالت سعفة اليد تكون العدوى فى يد واحدة فقط ‪.‬‬
‫في حالة إصابة كلتا اليدين يكون الطفح غير متماثل ‪.‬‬
‫في حاالت التهاب جلد اليد نالحظ زيادة وضوح عالمات الجلد ولكن في حالة سعفة‬
‫اليد المزمنة نجد هذه العالمات بيضاء بسبب القشور السطحية ‪.‬‬
‫في حالة سعفة اليد المؤثرة على ظهر تكون متواصلة مع إصابة راحة اليد ‪.‬‬
‫قد يكون للطفح حدود بارزة ‪.‬‬
‫مصاحبة بعدوى األظافر ( سعفة األظافر )‪ ,‬ولكن‬
‫في بعض األحيان نجد سعفة اليد‬

‫مرضية أخرى يمكنها أن تؤثرعلى األظافر ايضا‬

‫التشخيص ‪:‬‬
‫يتم التحقق من التشخيص بواسطة الفحص المجهري أو بزراعة عينة مكشوطة من‬
‫الجلد المتأثر بالعدوى ‪.‬‬
