Gabbay, MD, PhD Professor of Medicine, Penn State College of Medicine Until we meet again… Hold weekly improvement team meetings to plan/discuss PDSAs Develop a plan to collect any data you don’t have or correct any data you know is wrong Focus on implementing the first 4 critical changes (listed on your monthly report form): 1. Population alert 2. Use of template/flow sheet with embedded guidelines 3. Standing orders 4. Planned care at every visit Until we meet again… Identify diabetes patients not seen the past 6-12 months and test ways to reach out to them to schedule a visit and provide follow-up care Write out your PDSAs on the PDSA worksheets and summarize them on the monthly status report you submit to us Submit your data and status report to [email protected] by the 5th of the month Until we meet again… Review the data benchmarking reports compiled by the PA AHEC staff to see who is doing really well on key measures… and then call them or ask your facilitator to connect you Meet with your practice facilitator—they will be calling you Reflect on and respond to feedback from your practice facilitator Participate in once-a-month webinars Tell us what you need!
