• Employees earning $40,000 or less in base salary as of 9/30/2008 are awarded an additional $200 for rate relief. • You may be eligible to get as much as $1,100.

Annual Enrollment

• Enrollment is Oct 27 - Nov 10 (online) • Enroll if you wish to make changes or sign up for a flexible spending account in ’09. • Go online and confirm your choices and ’09 costs.

• JHU is going green.

This year faculty and staff will receive a reminder card for annual election, no other paperwork!

• No change to the Bargaining Unit Annual Election.

Annual Enrollment Checklist

• Access using JHED ID and password.

• Click “SUBMIT” to process your CHOICES, and remember to print your confirmation notice before logging off.

Dependent Age Limits

• Dependent children will be covered until the day before their 25 th birthday.

• NO student certification required.

• Proof of dependency required. Tax forms are the best means of verifying.

Life Events

• Change elections at Annual Enrollment or Life Event • Examples of IRS defined Life Event – Marriage – Birth – Change in employment status – Change in employment status of spouse

Short-term Disability Long-term Disability

Short-term Disability

• 14 calendar day elimination period.

• Pays 60% of salary ($1,000 per week max).

• Pays up to 11 weeks.

• If elected at annual enrollment, subject to pre existing conditions.

• Carrier: The Hartford

Long-term Disability

• Premium paid by JHU.

• One year waiting period (may be waived).

• Elimination period is 90 days.

• Pays 60% of salary; up to $10,000/month max.

• Benefits continue (medical, dental, life insurance).

• 10% of pre-disability monthly base salary contributed to a JHU 403(b) plan.

• Administered by WageWorks • Tax-free dollars = IRS rules • Use for eligible health and/or dependent care expenses – Health Care FSA (Medical/Dental) - $5,000/yr.

FSA Reimbursement

Using Your WageWorks Reimbursement Card

• Effective 1/1/2009, accepted at doctors’ offices, medical facilities, hospitals and qualified or IIAS certified merchants the same as a debit card; it allows you to pay for eligible health care products directly from your flexible spending account. • If the merchant is not qualified or IIAS certified, the reimbursement card cannot be used at that location. – Certified = sells greater than 90% flexible spending account eligible items • A list of the certified merchants can be found at www.sig-is.org

FSA Reimbursement

• When activating your reimbursement card online, update your email address in the contact information box since all WageWorks communication to participants is by email. • If you would prefer to receive paper statements, you can elect to do so at any time by logging in, selecting the FSA, then choosing View Account Statement.

Medical Plans

CareFirst BCBS

• Deductible $250 individual, $750 family • Inpatient Admission Copay - $250 per admission • Emergency Room Copay - $75.00 per visit • Coinsurance Plan pays 80% of allowed amount • Out-of-Pocket $1,500 individual, $4,500 family • Routine Care (Adult) – Physical and or OB/GYN exam – Vision screening (614-TEST)

EHP Classic

• Emergency Room Copay - $75.00 per visit • Inpatient Admission Copay - $250 per admission • Out of network Deductible $250/$750 Out-of-Network • Coinsurance – In-Network - Plan pays 80% allowed amount – Out-of-Network - Plan pays 70% allowed amount, balance billing • Out-of-Pocket Maximum (total of coinsurance) – In-Network - $1,500 individual, $4,500 family – Out-of-Network - $3,000 individual, $9,000 family

BlueChoice and Kaiser

• Emergency Room Copay - $75.00 per visit • Inpatient Admission Copay - $250 per admission • Must use network providers, PCP required • Employee with two or more children may elect coverage under the Parent/Child option -- less expensive than family coverage!

• Kaiser includes preventive dental care

Dental Plans

CareFirst PPO and CIGNA PPO

• Deductible – Out-of-network deductible dentist $50 individual, per year • Coinsurance – In-Network - Plan & Member %’s up to allowed amount – Out-of-Network – Member pays balance of charges after Plan %

United Concordia

• $5 copay for oral evaluation.

• Set fee schedule for services.

• Orthodontics – Pre-existing limitations!

– Member pays $2,900 (2 year case) • Must use primary care dentist.

• Referral required for specialist care.

• No claim forms to file!

403(b)

• There is no change to the JHU 403(b) plan.

• Review your investments closely • Contact us for information on roll-overs and transfers.
