Using the
Team Implementation Checklist

This checklist is designed to be completed by the PBIS Team once a quarter to monitor activities for implementation of
PBIS in a school. The team should complete the Action Plan at the same time to track items that are In Progress or Not Yet
Started items.

The TIC is designed for school teams to complete as a progress monitoring tool. The Coach or Team submits one TIC as
a summary of the team’s assessment of PBIS implementation per month or quarter.

Go to the PBIS Assessment website at: www.pbisassessment.org.
Click “Login” on the navigation menu at the top of the home page.
In the “School” tab; enter your six-digit School ID (account number).
Click on the blue “Login” button. You will be logged into your school’s PBIS
Assessment account. (Proceed to the correct survey instructions.)
Enter your School Name and School account ID below. The School ID will be
provided to you by your PBIS Assessment coach/coordinator.

5. On the “School Dashboard” locate the Team Checklist. Click on the arrow in the “Actions” column to access your
survey.
5. Begin completing the survey using the calendar to select the date the team competed the Team Implementation
Checklist. As you complete each page, click on the gray arrows at the bottom left.
6. There are a total of 22 questions in the seven sections of the Team Implementation Checklist. At the end of the
survey, click on the gray button “Submit Survey”.
7. You will receive a notice that your survey was submitted successfully.
8. Click on the “X” to leave the survey.
9. Click on the “Logout” button on the top right corner of the page.
Note: The Team Implementation Checklist (TIC) is designed to be completed by the PBIS Team once a quarter to
monitor activities for implementation of PBIS in school. The team should complete the TIC Action Plan (attached to
the downloadable PBIS TIC 3.1) immediately after completing the survey to track items that are In Progress or Not
Yet Started.
