Loss and Completion Reporting
Loss & Completion Reports contain activity that notifies the CLEC when Service Order activity is completed. The report data is sorted by Telephone Number.
Loss Report: The Loss report is issued when an order is completed or cancelled showing outward line activity, including a full or partially discontinued account. Completion Report: Is generated when an order is completed or cancelled. Completion reports can contain both Loss and Completion activity. © 2002 Qwest Communications International Inc.

What Reports and How Often?
Reports are generated for the following products: Resale Unbundled Loop Unbundled Switch Interim Number Portability (INP) Unbundled Network Element (UNE-P) Line Sharing Once the Loss & Completion Reports have been requested and established for a CLEC, they are produced and sent on a daily basis.

When are report Records Produced?
Completion Report data originates from Qwest competed or canceled Service Orders in support of the following activity : CLEC's LSR Another provider's LSR requesting service migration Qwest order to add Trouble Isolation Charges Qwest order to correct system records Loss Report data originates from Qwest completed or canceled Service Orders in support of the following activity: CLEC's LSR requesting service disconnect CLEC's LSR requesting product change Another provider's LSR requesting service migration © 2002 Qwest Communications International Inc.

Loss & Completion Report Delivery Methods
Loss & Completion Reports are sent to your assigned point of contact via the following transport mediums: NDM (Direct or Dial-In)- Is a mainframe product that can be used to transport a data file from one point to another.
Electronic Fax – Automated FAX delivery is a product that can transport a report format file from a server to a FAX delivery point via a BISCOM BAX gate transport interface. The Service Delivery Gateway (SDG) is a Web dial-up method of transport. The format can be either a report or a fixed flat file format. © 2002 Qwest Communications International Inc.

Report Changes Initiated By CMP
CMP Change Requests (CR) can be issued by the CLECs to request changes to the Loss & Completion Reports. The release dates for the Loss & Completion Reports are posted to the CMP OSS calendar. © 2002 Qwest Communications International Inc.

Recent Changes to the Reports
Edits were discovered in the Qwest internal systems that caused occasional processing inconsistencies for the Completion Report. An enhancement to remove these edits was deployed April 25, 2003.
The range of numbers reported behind the SNR FID were not being displayed on the Loss & Completion Reports. Only the first number appears on the report. Qwest enhanced an internal system to provide all numbers in the range to the Loss & Completion report system. This was deployed March 30, 2003. © 2002 Qwest Communications International Inc.

Upcoming Changes to the Reports
SCR093002-01
- Change loss report to include only external losses Currently the loss reports include telephone numbers that have outward activity. This SCR requests that the criteria for the loss report be changed to only report losses due to conversions to another provider. Scheduled to deploy June 30, 2003.
SCR022703-12
- Line Loss Notification- 836 EDI This CR was prioritized as number 13 in the IMA 14.0 vote. If this CR is packaged into the IMA 14.0 release the Loss Information will be available via IMA GUI or IMA EDI. The IMA 14.0 release is scheduled to deploy on December 9, 2003.
