Thursday, April 30, 2015: Deadline – end date for all activities Hospitals/partners − Friday, May 8, 2015: Submit scorecard to OPO/DLA with all activities conducted through April 30 and total Phase IV new donor registrations OPOs/DLAs − Friday, May 15, 2015: Deadline – Forward all scorecards, reporting all points earned and total Phase IV new donor registrations from each partner to [email protected]

Recognition Timeline

• • • All OPOs, DLAs, and hospital associations who have participated in Phase IV will be formally recognized by HRSA June 2015 – HRSA begins recognition for Campaign achievements Honorees posted on organdonor.gov

• • Hospitals reaching bronze, silver, or gold status receive special certificates and web badges, provided to their OPO/DLA partners for distribution/presentation News release template provided for DLAs, OPOs, hospitals, and hospital associations to announce involvement, achievement, and outcomes
