Important Information About
6th Grade Immunizations
Before 6th grade, each student must have:
 An updated Immunization Form #3231
 Two doses of Measles vaccine, two doses of Mumps
vaccine, one dose of Rubella vaccine, laboratory proof
of immunity against each of these three diseases
 Two doses of Varicella (chicken pox) vaccine or
documentation of disease or laboratory proof of
immunity. At the time your child entered school in
kindergarten, only one dose was required.
 Check newspapers and websites for a Back to School
Health Fair offered before school starts.

Criteria for Placement
Course placements at Mill Creek are
assigned based on criteria set forth by
the Cherokee County School District
(CCSD) which includes the scores your
child had on the 4th GA CRCT, ITBS,
and classroom performance. The
curriculum is rigorous in comparison
to elementary school with more work
to complete in class and at home.

PE is yearlong for most students

Music students will have PE on a quarterly basis

Students are required to dress out according to the PE
dress code

The dress code includes athletic shoes, regular t-shirt and
athletic shorts that must be longer than five inches above
the knee.

PE uniforms are available to purchase during
walk-throughs in July

PE lockers are available to purchase during your child’s
PE class

Health is a quarter long connections class that focuses on
mental, physical, and social health, nutrition and drug
prevention education
