Implemented Smoking as a Vital Sign
(SVS) into the systems of 7 of our
associated health centers
– 17 adult medical clinics
– 2 pediatric clinics
– 8 school based health centers
– 1 teen clinic
Our Focus

practices and knowledge base of the
Health Care Team (all levels of service
providers)

