CDSS requires all THP-Plus Providers have a
THP-Plus Plan on file.
◦ Counties are responsible for certifying that Provider
Plans meet all the requirements set forth in the
regulations.

Provider Plans are the foundation for the
THP-Plus County’s THP-Plus Program.

Provider Plans feed into the overall County
THP-Plus Plan.




Provider Plans need to be certified by the
County prior to THP-Plus services being
provided.
Provider Plans need to include all required
information.
Provider Plans should be reviewed and
updated annually.
Provider Plans must be kept on file at the
County Social Services Office.


CDSS regulations require specific items be in a Provider
Plan.
