Hazard severity is an assessment of the worst potential consequence, defined by degree of injury or property damage, which could occur Probability is the likelihood that an identified hazard will result in a mishap, based on an assessment of such factors as location, exposure in terms of cycles or hours of operation, and affected population. These levels are derived from MILSTD-882D, Standard Practices for System Safety. Defense Standards are also used by other non-Defense government organizations, technical organizations, and industry Environment, Safety and Health CXI Final Instrument Design Review, June 3 2009 6 6 Michael Scharfenstein [email protected] Hazard Consequence Rating Environment, Safety and Health CXI Final Instrument Design Review, June 3 2009 7 7 Michael Scharfenstein [email protected] LUSI CXI Configuration Hazards Analysis Radiation Fire Protection Electrical Seismic Chemical Thermal/Cryogenic Mechanical Aerosolized Nanoparticles Environmental Insults Optics & Diagnostics Air Emissions Waste Material Disposal Diagnostics Magnetic Oxygen Deficiency Vacuum / Pressure Laser Design Hazards Pinch points Sharp corners Environment, Safety and Health CXI Final Instrument Design Review, June 3 2009 1 micron KB Reference Laser 8 8 1 micron Sample Environment 0.1 micron KB & Sample Environment, Michael Scharfenstein [email protected] Conclusion Integrated Safety and Environmental Management Following SLAC ISEMS Process SLAC ES&H Manual Safety Oversight Committee Following LCLS ES&H management practices LCLS Requirement, Specification and Interface Documents Work Authorization Hazard Analysis Report Approved by DOE Environment, Safety and Health CXI Final Instrument Design Review, June 3 2009 9 9 Michael Scharfenstein [email protected]
