• All attending in a group, a roster must be filled
out and signed by a supervisor.
• In addition to the attendance roster each
attendee must complete and return an
evaluation form to receive CEU’s.
• A certificate of attendance indicating the CEU’s
will be emailed.
• Please return roster and evaluation forms to
