주위에 생긴 두드러기를 주소로
내원하였다.

모유수유 중 엄마의 우유 섭취 후에 피부에
발진이 생겼고, 생후 6개월경 분유 먹은
후 전신에 두드러기와 가려움증 있었다.
이후 엄마가 우유 등에 대한 제한식이를
하면서 시행한 모유와 저알레르기성 분유
의 혼합수유에는 이상소견이 관찰되지 않
았다.

환아는 우유 및 계란에 대하여는 제한식이
를 지속하고, 이유식은 60분 정도 충분
히 가열한 미음으로 진행하면서 피부 발
진 및 두드러기의 발생은 더 이상 없었
다.
5개월/여아
주소: 이유식 분유 한 스푼을 먹고 발생한 구토, 기면, 설사
검사
말초혈액 호산구수: 120/mm3 ,Total IgE: <18.0 IU/mL
MAST: all (-)
Skin prick test: 계란, 우유, 생선, 새우에 음성, 밀에 약양성
CAP: 우유, 콩, 계란 흰자, 땅콩, 밀, 쌀, 새우, 메밀 모두에 음성
Provocation (쌀미음): 수유 후 1-6시간에 걸쳐 구토, 청색증, 기면, 기관
지천명, 설사 등의 증상
유발시험 후 6시간에 시행한 상부 위장관 내시경 검사: 십이지장
구부 점막의 붉은 부종성 병변, 현미경 소견에서 아전 융모 위축
과 고배율 시야 당 약 10개의 호산구 침윤 소견
