皮膚炎(Dermatitis)分類注意事項：
一、按照醫師處方指示服用藥物所引起的副作用。
主診斷：皮膚炎代碼
附加診斷：治療性外傷原因代碼( E930.x-E949.x）
EX..Toxicoderma 為藥物引起如何編碼？
code：695.0+E-code(E930-E949)
皮膚炎(Dermatitis)(續)
二、因意外性之藥物中毒或不正確的使用藥物所引起
皮膚炎。
 主診斷：中毒碼（960.x-969.x）
 附加診斷：藥物引起之皮膚炎代碼，及意外中毒
之外傷原因代碼 ( E850.x -E869.x )。

皮膚潰瘍 (一)
 皮膚潰瘍(Ulcer ) 分類注意事項：
一、皮膚潰瘍(Ulcer )併壞疽(Gangrene)時，編兩個代碼潰
瘍 ( 707.x ) 及壞疽 ( 785.4 ) 。
二、糖尿病與足部潰瘍
 糖尿病患者，足部因感染而潰瘍，則潰瘍不一定視為
糖尿病合併症。
 如果足部潰瘍是因細菌感染而引起，而與糖尿病無關
時。
主診斷：707.1( 足部潰瘍 )
附加診斷：致病之細菌名稱及250.0x
( 糖尿病無合併症 )
皮膚潰瘍 (二)
三、糖尿病合併神經病變或糖尿病合併週邊血管疾病，
須編碼為糖尿病合併症。
主診斷：250.6x( 糖尿病合併神經病變 )或250.7x
(糖尿病合併週邊血管疾病 )
附加診斷：707.1( 下肢潰瘍 )，如併有壞疽須再編
785.4。
四、患者皮膚潰瘍確實是與糖尿病相關，但不能判斷是
神經病變或週邊血管疾病所引起的。
☆主診斷：250.8x ( 糖尿病合併其他病變 )
☆附加診斷：707.1 ( 下肢潰瘍 )
皮膚潰瘍 (三)
褥瘡併薦廔管發病時，代碼 707.0。因薦
廔管為褥瘡的病程之一部分，屬同一病況，故
不需另外編碼。
 pressure sore with二度infection
code: 707的代碼，若有特別提及何種菌
種感染加編菌種碼。(病管協會Q&A)
蜂窩組織炎(一)
蜂窩組織炎(Cellulitis)注意事項：
一、因表淺性傷口或凍傷引起蜂窩組織炎時，需
編二個代碼，一個是損傷代碼，另一個是蜂
窩組織炎代碼，須注意選取住院之主要原因
為主診斷。
1.當入院是治療開放性傷口時
主診斷：開放性傷口有併發症
附加診斷：蜂窩組織炎
2.當入院是治療蜂窩組織炎時
主診斷：蜂窩組織炎
附加診斷：開放性傷口有併發症

例
Leg 受傷後使用外用草藥或無治療，因有
Cellulitis ,wound infection ，入院時因
cellulitis ，故以cellulitis of leg 682.6
當主要診斷，891.1為次要診斷
因 open wound of leg入院，在入院後才出現
cellulitis ，則wound infection of leg
891.1當主要診斷，682.6為次要診斷
蜂窩組織炎(二)
二、蜂窩組織炎併膿瘍(Cellulitis and abscess)
代碼681.xx及682.x，若得知感染的細菌名稱，
需加編附加碼。
三、Cellulitis sometimes occurs in areas other
than skin and subcutaneous tissue

案例
 Lower leg of cellulitis due to previous of
contusion？
code:682.6+906.3(cellulitis 為contusion 之
late effect所致)。
 患者10天前下肢紋身(tatto)，此次入院診斷
Lower leg cellulitis with folliculits due
to tatto？
code:682.6+704.8即可，但刺青仍舊存留，可加編
代碼709.09。(因tatto屬自願性侵襲處置，但
非醫療性或必要之處置，無合適的Complication
代碼可呈現。
壞疽性膿皮病(pyoderma gangrenosum)
最常見的原因
潰瘍性大腸炎 556.9
局部腸炎 555.9
診斷為Pyoderma gangrenosum需編病因及膿皮病
兩個代碼，且應以病因代碼為主診斷。
Ex.患者診斷為Pyoderma gangrenosum(壞疽性
膿皮病)及ulcerative colitis，ulcerative
colitis為主要診斷，686.01為附加診斷。
手術及處置分類規則(一)
局部切除皮膚或皮下病灶
–代碼： 86.3
根除性病灶切除
–代碼： 86.4
•如有淋巴切除須加編40.3代碼
手術及處置分類規則(二)
清創術(Debridement)
指清除外傷或感染的傷口或移除異物，
感染及壞死的組織。
–86.22 Debridement ( 清創術 ) ：
指外科切除或用雷射移除壞死組織。
– 86.28 Nonexcisinoal debridement
(非切除性清創術) ：
指僅以刷除、灌洗及清洗小範圍的壞死組
織。
手術及處置分類規則(三)
皮膚移植(Skin graft)
 蒂狀皮瓣(pedicle flaps)分類：
–前移皮瓣(advancement flaps)，代碼86.72 。
–移位式皮瓣(Transposed flaps)，附著式皮瓣或轉移
式皮瓣，代碼86.74。
–旋轉式皮瓣(Rotation flaps)，代碼 86.70 。
–管式蒂狀皮瓣(tube pedicle flaps)，代碼 86.70 。
–島狀式皮瓣(island flaps)，代碼 86.70 。
–Z型整型(Z plasty)，代碼 86.84 。
–人工皮(application of integument) 是一種化學合
成的人造皮，減少肉芽組織生成，代碼86.67。
手術及處置分類規則(四)
處置類目碼86之下Exclude之註解說明規定：
切除肛門、耳、眉、眼瞼、女性會陰、嘴
唇、鼻、陰莖、陰囊、陰唇等部位的皮膚
手術並不包括於處置類目碼86中，而依說
明另編代碼。
例：左側乳房膿瘍施行incision and drainage 及
debridemet，編代碼85.0(mastotomy)；當施行
切開引流同時做debridemet時， debridemet已
包含在乳房切開中，故不另予編碼，只編85.0即可。

開放性傷口(open wound )
 第四位碼辨識開放性傷口的合併症(Complication)，
此合併症適用於延遲癒合 (Delayed healing)、
延遲治療(Delayed treatment)、異物滯留
(Foreign body)、重大感染(Major infection)。
–Open wound by site with infection，應視為該部
位有合併症，編碼時給該部位之合併症代碼。
ex.Traumatic amputation of thumb ,delayed
healing
code: 885.1
–外傷後之傷口後續性感染，編碼為958.3 。
開放性傷口(續)
 挫傷(Contusion wound)應看是否有開放性傷口，
無open wound 則依部位編挫傷碼，若有開放性傷
口，因開放性傷口比挫傷嚴重，故依部位編開放性
傷口碼。
 同一部位Contusion 與abrasion 同時發生時，依
coding 原則仍編Contusion，編碼原則仍以深層之
傷害為主。

當原發損傷深及血管或神經
–主要診斷：血管或神經部位之診斷
不正確用藥
不正確用藥(如過量、給錯、吃錯)及
化學物質所致之不良反應，常見之
關鍵字為Overdose, intoxication
完整的編碼為：
中毒碼 + 徵狀碼 + 外傷原因代碼

正 確 用 藥
 正確用藥所致不良反應(如副作用、藥物過敏、
特異體質反應..等)，可直接由不良反應本質或
其所表現出來的狀況編碼，如胃炎、蕁麻疹、
眩暈等；這些代碼並不歸屬於960~ 979的分類代碼
中。
 正確用藥之編碼原則
–藥物治療引起之過敏，其分類代碼為796.0+E930~E949
–有明確之症狀時，編寫徵狀碼 + 外傷原因代碼(E-code)
–藥物依賴，不能當作中毒的狀況編碼，但過量使用時則
以Poisoning編碼
Ex.Rash due to penicillin 693.0+E930.0
內外科合併症(一)
定義
–入院後經手術或其他醫療處置所引起
之合併症
–創傷之早期合併症，包括創傷後休克、
創傷後傷口感染、創傷性皮下氣腫
內外科合併症(二)
分類原則
–以該外科手術後之合併症之狀況為關鍵字
•Postoperation(手術後的)
•Due to surgery(伴隨手術後)
–以該手術後之合併症(Complication)
為關鍵字，代碼996 ~ 999
內外科合併症(三)
代碼特性
–有些狀況縱使已明示為手術後之合併症，但仍
然可以歸納於該系統疾病代碼表達，不必再加
編E-code。
Ex.tracheostomy complication code:519.00
–手術後之併發症，若未被歸類在代碼001-799
者，可編於代碼996 ~ 999中
–因放射治療所造成之合併症代碼 990

)
代碼998 描述手術引起之其他併發症
–代碼998.1 醫療程序中出血或血腫之併發症
–代碼998.2 醫療程序中意外刺穿或裂傷

•不包括 代碼674.1x 產科剖腹產傷口裂開
代碼674.2x 自然產會陰縫合傷口裂
多處創傷(Multiple Trauma)
 指於同一事件所造成之多處創傷。
 遇診斷為“multiple injuries” ，若於病
歷上未明示其創傷部位，則編“multiple
injuries”之代碼，若於病歷中找出創傷
之各別部位，則應編各別部位之創傷代
碼。
 於多處創傷中取最嚴重之創傷當主診斷，如果
診斷描述不清楚必須請教醫師。
 若兩個或以上同樣嚴重之診斷，則取與治療
(處置)最具關連之診斷當主診斷。
