Applications MUST Include
The CORRECT form
Proof of Grades (report cards)
Service Hours forms (with outside service
letters, if applicable)
Service Hours
 Must do at least 5 hours per semester from now on
 You do not have to do extra hours to make up for past
applications
 School-based service is preferred
 For outside service, you need to get it pre-approved (i.e.
don’t do it until I say it’s okay)
 You need an official letter from the organization with:
 the date service was provided
 description of service
 hours completed
 name and signature of the contact person.
 The coordinator may not be your parent or a relative.
 Attach the letter with your application
Service Opportunities
 The Teacher Job Board
 School Organized: Beach Cleanup
 Saturday May 23rd from 8:30-11:30
 MUST turn in Waiver and Permission Slip by 5/15/15 to be included
 FREE, but you need to provide your own transportation
 Carpooling is encouraged 
8th Grade Benefits
 All members during their 8th grade year get a goal seal on their
promotion certificate
 Members for at least three semesters (3 or 4 punches in their card)
are Honor Members
 Honor Members receive a gold pin, a special certificate, recognition at
the 8th Grade Awards Night, and preferential seating at the promotion
ceremony
 Current Candidates who are relying on the 2nd semester of 8th grade
need to turn in complete applications by May 29, 2015 to receive honors
other than the pin and certificate
CJSF Officers
 Current 7th graders can run for officer positions- President,
Vice-President, Secretary, and Treasurer- at the start of the
next school year.
