appropriate public agency within the
system for:
Evaluation and assessment, or
As appropriate, the provision of
services.

Hospitals, including prenatal and postnatal
facilities;
Physicians;
Parents;
Day care programs;
Local educational agencies;
Public health facilities;
Other social service agencies; and
Other health care providers.

Timelines



Once the public agency receives a
referral, it shall appoint a service
coordinator as soon as possible.
Within 45 days after it receives a
referral, the public agency shall:
Complete the evaluation and
assessment activities, and
Hold an IFSP meeting.
