Team Around The Child

The Named Person /Lead Professional is responsible for calling a TATC Decides who is invited in consultation with the child/young person and their family Has preparatory discussions with the child and her/his family Discusses, negotiates, designs and reviews the Single Child’s Plan Can appoint or change a Lead Professional at Levels 2 &3

Team Around the Child

Level 1 : When the Named Person assesses that there are wellbeing concerns that cannot be resolved within mainstream universal services. Support is single agency at this stage Level 2 : assessment indicates insufficient resources within community or class/post school setting to resolve wellbeing concerns. Likely to be multi agency at this stage and a Lead Professional will be required to co-ordinate the Child’s Plan that will emerge.

Level 3 : A “voluntary” integrated approach has not resolved the issues. Wellbeing concerns are such that a referral to SCRA or to Options is being seriously considered.

The single planning process supporting the single Child’s Plan Ensures continuity and co-ordination of support for the child and her/his family.

Ends the practice of numerous assessments covering similar information from numerous agencies.

Gives the child/young person and their family an identified NP/LP to relate to. They don’t have to “tell my story over and over again”

Ensures all partners to the plan work together to achieve agreed outcomes. Children, young people and their families can be partners to their own plan. Doing things with people rather than to them The plan is asset based and solution focussed : and has a contingency plan if required Ensures continuity if the Lead Professional changes. The new LP will assume responsibility for the plan.
