בטן -רגישות דיפוזית ,תנגודת ,ריבאונד
אבחנה משוערת ?

Diabetic Ketoacidosis
Acute, life-threatening metabolic acidosis in Type 1 DM,
less frequent in Type 2 DM with intercurrent illness
(infection, MI, surgery)
Etiology
• Presentation of type 1 diabetes
• Infection, trauma (stress)
• Missed insulin injection
Mortality rate: ~ 0.5%

Clinical Presentation
Anorexia, polydipsia and polyuria, followed by
stupor/ coma
Dehydration
Sings of dehydration ( HR, postural
hypotension...)
Nausea, Vomiting, Abdominal pain & tenderness
Kussmaul breathing with acetone “fruity ” odor
Normal or low temperature, unless infection

Diagnostic Criteria for DKA:
Hyperglycemia (usually >250 mg/dl)
Ketosis (ketonemia or ketonuria +++)
Acidosis (pH<7.3, HCO3<15mEq/L)

Therapy of Diabetic Ketoacidosis (1)
Monitoring (ICU)
• Draw blood for electrolytes, glucose (glucometer) & astrup
• Hemodynamic status, astrup and biochemistry every 1-2 h
Fluids
Normal saline 1 L/h
If pH <7.0 or bicarbonate <5 give bicarbonate (100 mmol)
Insulin
loading” ~ 0.3 U/Kg
Then ~ 0.15 U/Kg/h (IV drip)
Adjustment of the dose according to astrup and blood glucose

Therapy of Diabetic Ketoacidosis (2)

Glucose < 250 mg/dl, switch to 1/2 N/S+5% glucose
Add KCl (10-30 mmeq/L)
Avoid rapid decline of blood glucose !!!

היפוגליקמיה
היפוגליקמיה קלה
• אין צורך לתת גלוקוז לתוך הוריד
• יש לתת גלוקוז או סוכרוז 15-30גרם דרך הפה בצורת
טבלית סוכר או שתייה ממותקת
• כאשר לא ניתן לתת ,אין תגובה לטיפול פומי או
היפוגליקמיה חוזרת (סולפונילאוראה ,הפרעות ספיגה,
גסטרופרזיס) ,יש לתת עירוי של 5-10%גלוקוז דרך
הוריד

היפוגליקמיה -המשך
היפוגליקמיה קשה (חולה הנזקק לעזרתו של אדם נוסף)
• 20%גלוקוז דרך הוריד 60-120מל’ בולוס ראשון ,אם אין
שיפור במצב יש לחזור על הבולוס עד להתעוררות
• בהמשך עירוי של 5-10%גלוקוז .יש לשמור את רמת
הגלוקוז בערכים של 100-150מג’/דל’
• תמיסות מרוכזות של גלוקוז ( 10%ויותר) יש לתת לוריד
פריפרי גדול
• אם אין אפשרות לתת גלוקוז דרך הוריד יש לתת גלוקגון 1.0
מג’/דל’ בזריקה לתוך השריר

אבחנה של סוכרת
נוכחות תסמינים של סוכרת עם תיעוד של בדיקת גלוקוז
מקרית שערכה גבוה מ 200-מ"ג לדציליטר (ללא קשר
למועד הארוחה האחרונה) .התסמינים האופייניים של
המחלה כוללים שתן מרובה ,שתייה מרובה ,וירידה
בלתי מוסברת במשקל.
רמת גלוקוז בצום של 126מ"ג לדציליטר לפחות .צום
מוגדר כשמונה שעות לפחות ללא צריכת כל מוצר מזון
בעל ערך קלורי( .בדיקה חוזרת)
רמת גלוקוז של 200מ"ג לדציליטר לפחות לאחר
העמסת סוכר פומית ( 75גרם סוכר יבש מומס במים,
על פי הנחיות ארגון הבריאות העולמי)( .בדיקה חוזרת)
( .HbA1c>6.5%בדיקה חוזרת).

A1C ≥6.5%

Fasting plasma glucose (FPG)
≥126 mg/dL (7.0 mmol/L)
OR
2-h plasma glucose ≥200 mg/dL
(11.1 mmol/L) during an OGTT

*In the absence of unequivocal hyperglycemia, result
should be confirmed by repeat testing.
