El éxito en la evaluación por

del hígado
depende de
• Alta resolución espacial
• Alto contraste entre hígado y lesión
• Habilidad para distinguir la patología
significativa de la no significativa

LESIONES FOCALES
QUISTE
SIMPLE
LESIONES BENIGNAS:
Muy comunes,
habitualmente
solitarios
QUISTE
SIMPLE
Pueden asociarse a enfermedad poliquística
renal
Son dilataciones quísticas de conductos
biliares aberrantes en el higado (complejo de
Meyenburg) que no se conectan con los
conductos biliares normales
Complicaciones: crecimiento, hemorragia,
ruptura, torsion
QUISTE
SIMPLE
LESIONES BENIGNAS:
QUISTE SIMPLE
• Multiple cysts are visible on cut surface of
liver; the cyst walls are thin, translucent,
and grey. This is from a case with
polycystic disease; note the small green bile
duct hamartomas in the surrounding liver
QUISTE

LESIONES BENIGNAS:
Pared fina

atenuacion lateral.
TC:
0-10UH.
La pared no refuerza.
RM:
Hipo en T1, hiper en T2 (más hiper en TE

nódulo displásico de alto grado nódulo displásico con
focos de HCC HCC pequeño HCC franco
Puede presentarse como:
múltiples nódulos de distinto tamaño

in a chronic viral hepatitis; the tumor, which
had a predominant acinar architecture,
produced abundant bile

COLANGIOCARCINOMA
LESIONES
MALIGNAS:
Nace del epitelio biliar ductal (adenocarcinoma)
De acuerdo en elColangioCa
sitio de origen:
Periférico: pequeños conductos interlobulares (60-70
años)
De conductos mayores: conductos hepáticos
Hiliar: de la bifurcación (Tumor de Klatzkin)
Factores predisponentes
No relación con cirrosis (25% de los ColangioCa)
Sobrevida a 3 años de 3%
El intrahepático se presenta como:
Gran masa solitaria
Múltiples masas confluentes
Forma difusa
Patológicamente muestra abundante esclerosis
Raro invasión vascular

Caroli, quiste de colédoco)

Atresia biliar congénita
Exposición al thorotrast
COLANGIOCARCINOMA
LESIONES MALIGNAS:
ColangioCa
• Peripheral cholangiocarcinoma secondary to remote
Thorotrast exposure; a dominant mass replaces the entire
left lobe, while the multiple secondary nodules in the right
lobe mimic metastasis
• The plain film of the abdomen still reveals residual
Thorotrast in the spleen and peripancreatic lymph nodes
COLANGIOCARCINOMA
LESIONES MALIGNAS:
ColangioCa
• Several larger triads within the liver were expanded and
fibrotic; tumor was present within the ductal system, but
was not invasive
COLANGIOCARCINOMA
LESIONES MALIGNAS:
ColangioCa
• A malignant stricture caused by tumor encasing the hepatic
ducts as they ramify through the liver
COLANGIOCARCINOMA
LESIONES MALIGNAS:
ECO: hiper homogeneo.
También hipo e iso,
ColangioCa
TC:
sin: hipo, irregular. A veces hiper central por mucina
