TO GAIN CLEARANCE TO TURN. THE GAS PEDAL
STUCK SLIGHTLY CAUSING THE VEHICLE TO LURCH,
PINNING AND CRUSHING THE SPOTTER BETWEEN THE
FORKLIFT AND A POLE RESULTING IN FATAL INJURIES.
CITATION: FAILURE TO MAINTAIN CLEARANCE BETWEEN A FIXED
OBJECT AND THE VEHICLE.
