The Office of Disability Support Services has approved the accommodations below based on documentation
verifying the existence of a disability and the review of their documentation by DSS to evaluate the student's
needs.
In accordance with Section 504 of the Rehabilitation Act and the Americans with Disabilities Act, the purpose of
these accommodations is to give students an opportunity to demonstrate mastery of course content and
information being tested by minimizing or eliminating the impact of the disability. It is not to give students with
disabilities an unfair advantage.
It is the students’ responsibility to request accommodations and it is the responsibility of DSS to provide them.
Faculty are an integral part to this process as well. We strongly encourage the student and faculty member
to meet privately to discuss their accommodations and to meet as early as possible in the semester.

Faculty members should contact DSS if they feel that:

any of the academic accommodations may result in a fundamental alteration of the course;

the student’s requests are unreasonable;

they are uncertain about a request by the student for an additional accommodation not listed in the letter; or

they have not been notified in a timely manner
STUDENT GRIEVANCE PROCEDURE: Conflict and disagreement sometimes occur in accommodation matters. Such
conflict and disagreement can best be solved through dialogue between parties. Therefore, as a first step in the grievance
procedure, DSS encourages students to talk directly with DSS staff or with the faculty member when there is a concern
about classroom or testing accommodations. If such dialogue is unsuccessful, DSS has procedures for addressing
grievances. Those procedures may be utilized when students do not agree with the academic accommodations that are
offered by DSS, or when an instructor does not adhere to the academic accommodations approved by DSS. This process
begins by contacting the DSS Coordinator and/or Director. The grievance procedure can be found on the DSS website.

Inform students of office location and procedures for requesting accommodations.
Accept and evaluate verifying documentation.
Maintain accurate documentation for each student.
Approve appropriate accommodations.
Provide notification to individuals as directed by the student.
Protect student’s privacy and confidentiality.
Provide equal access to programs and services.
Responsibilities
Provide courier service to pick up and deliver exams.
Maintain integrity with the exams and exam process.
Report any suspected academic dishonesty to the appropriate faculty member/department.
STUDENT RESPONSIBILITIES
General







Responsibilities
Meet the essential qualifications and institutional standards.
Disclose the disability in a timely manner.
Provide appropriate documentation to DSS.
Inform DSS of barriers to a successful education and accommodation needs.
Talk with professors about accommodations. Bring the accommodation letter and any additional documents (i.e. notetaker
announcement, recorded lecture policy) to faculty member.
Discuss with the faculty member how the accommodations are going to be implemented and explain about the situation,
particularly testing accommodations.
Request accommodations each semester. Accommodations cannot be granted retroactively.
Testing Responsibilities

Before each test during the semester, students will need to confirm with their professor that they plan to use the
accommodations.

Students needing to take their test through the DSS testing process must complete the student section of the “TEST
REQUEST FORM” before presenting the form to the professor/instructor. The student is responsible for returning all copies of
the completed “TEST REQUEST FORM” to the DSS office.

The “TEST REQUEST FORM” must be turned into Disability Support Services at least five (5) business days (two (2) weeks
before the first day of the final exam period) before the exam is to be taken.

Students must arrive on time to take the exam. If students are more than 15 minutes late, the amount of time late will be
deducted from the allotted exam time.

Students must adhere to the University’s honor statement. Students suspected of academic dishonesty will be referred for
disciplinary action.
FACULTY RESPONSIBILITIES
General Responsibilities

Make sure that each course, viewed in its entirety, is accessible, e.g., content, texts and materials, assessment method, online instruction, time requirements.

Support and implement reasonable accommodations as identified in a letter from DSS and presented by the student.

Consult with DSS if requests conflict with course objectives/requirements.

Maintain CONFIDENTIALITY regarding accommodations.

Submit book lists to bookstore as early as possible so they may be converted to accessible format.

Include announcement on syllabus directing students to DSS if they have need for an accommodation related to a disability.
Testing Responsibilities

Verify that the student is registered with the Office of Disability Services by referring to the letter sent by the Office of Disability
Services outlining appropriate and approved accommodations. No accommodations are to be allowed until a letter is received.
Only the accommodations listed on the letter will be granted.

Thoroughly and legibly complete the Professor/Instructor part of the “TEST REQUEST FORM” and return the form to the
student. The professor must indicate on the form where to return the exam upon completion.

If the courier method of deliver has been chosen, the professor must have the exam ready to be picked up by the Disability
Services courier at least 24 hours before the exam is to be taken. Disability Support Services will provide a secure envelope in
which to place exam(s). If the professor/instructor chooses to utilize other delivery methods, they must adhere to the following:
the professor/instructor may deliver the exam(s) to 201 Pryzbyla Center, fax the exam(s) to (202) 319-5126 (only during office
hours and after contacting the office), or e-mail the exam(s) to: [email protected].
DISABILITY SUPPORT SERVICES TEST ACCOMMODATION REQUEST FORM
Tests can be scheduled Monday – Friday from 9:00 a.m. to 5:00 p.m. Completed Test Accommodation Request Forms must be delivered to DSS at least 5 business
days prior to the test or 1 month prior to final exams. Test/exam administration is not guaranteed for requests not submitted in a timely fashion.

Student’s Process & Instructions:
1. Complete the Student Information to the right.
2. Bring the form to your instructor. It is recommended that you
meet with them in person and wait while they complete the form.
3. Your instructor will complete the sections for Instructor and
Test Information to the right.
4. Return the entire completed form to DSS for approval within
the appropriate time frame.
5. DSS will complete the Accommodation Approval Section, sign
the form, and return the pink and gold copies to you. DSS will
schedule the exam at this time.
6. Return the pink copy to your professor. Keep the gold copy.
7. Your instructor will provide DSS with a copy of your exam at
least 24 hours before your test. Call DSS to confirm its receipt
and exam arrangements the day before your test.
8. Arrive at your testing room 10 minutes prior to starting your
exam. If you are late, your test time will not be extended.
9. DSS will deliver your completed exam to your professor within
24 hours of your exam’s end time.
Instructor’s Process & Instructions:
1. The student will complete the Student Information to the right
and bring the form to you.
2. Complete the Test Information, Alternate Testing Information,
and Exam Pick-up and Delivery Information sections to the right
while meeting with the student. Please do not remove any
sheets.
3. The student returns the entire completed form to DSS for
approval within the appropriate time frame.
4. DSS will complete the Accommodation Approval Section, sign
the form, and return the pink and gold copies to the student.
DSS will schedule the exam at this time.
5. The student will bring you the signed pink copy.
6. Provide DSS with a copy of your exam at least 24 hours before
the exam time. Exams may be faxed, e-mailed, delivered to DSS
or picked up by a DSS courier. See the back of this form for
contact information.
7. The student will confirm the exam’s receipt and exam
arrangements the day before the test.
8. The student will take the test at TBA
9. DSS will deliver the completed exam to you within 24 hours of
the exam’s end time.
