Osteoarthritis, Joint Stress and Injury Prevention in Livestock Production • The National AgrAbility Project reported the top-ranked enterprise among AgrAbility clients to be livestock production (26.5%), and the top-ranked disability to be arthritis (12.5%). • Arthritis is the number one disability-causing disease in America, with nearly 50 million American’s having a diagnosis of at least one form of arthritis. Arthritis is an increasing concern for farmers and ranchers. • The occupational risks associated with raising livestock are numerous, and are directly related to osteoarthritis pain and decreased functionality. Arthritis Defined… • The term arthritis refers to an “inflammation of a joint”. • It is used to refer to over 100 rheumatic diseases that are characterized by problems in and around joints. • Most are chronic and have no cure. A Nation In Pain • 50 Million people in the US have doctor diagnosed Arthritis. • By the year 2030, an estimated 67 million or 25% of the projected total adult population will have arthritis. • Arthritis is the most common cause of disability in the US workplace, limiting nearly 21 million Americans at their job. • More than half of the individuals who have arthritis are younger than 65 years old. Risk Factors Risk factors a person cannot control: • Age 65 + • Gender • Genetic/Family history Risk factors a person can control: • Excess weight • Joint support • Activities and sports • Occupational hazards Signs and Symptoms • Persistent pain with no relief • Stiffness, swelling, redness or heat, locking of the joint • Difficulty performing daily activities • Difficulty moving the joint Osteoarthritis Most Common Form Seen in Agricultural Workers • Wear-and-tear, or degenerative, arthritis that generally affects weight-bearing joints or joints that have been stressed or damaged by injury. • OA is not a consequence of aging- it is a progressive degeneration of cartilage. • Affects only certain joints, often affects joints on ONE side of the body at first, but can affect multiple joints at once. • Affects the range of motion of the joint, pain/tenderness, may cause some swelling. Effects of OA on Living and Working • Farmers and ranchers with arthritis are faced with many obstacles that limit their ability to continue farming. Like farming, arthritis is often a 24hour, 7-day a week occupation. • As hard as it is for a farmer to change routine, life-style changes can be beneficial. And not all change has to use technology. Common Sources of Joint Stress and/or Injury in Livestock Production • Extension • Vibration • Compression • Repetition • Lifting/Carrying • Climbing/Bending Animal Husbandry • Breeding – Animal temperaments, chance of injury • Foaling/Calving/Lambing/Farrowing – Stress and worry, fatigue – Physical work and joint strain (posture, back pain) • Weaning – Animal temperaments, chance of injury – Use of physical strength to separate animals Production and Processing • Finishing for Market – Feedlots (number of animals in pens) • Processing – Dehorning, Clipping Needle Teeth, Castration, Branding, Ear Tagging, etc. • Poultry – Egg Collection • Dairy Operations – Milker’s Knee • Sheep/Goat Production – Shearing, Trimming Hooves Production and Processing Recommendations • Livestock chutes • Raised tables for shearing, hoof trimming • Support poles for leverage when bending and kneeling • Anti-fatigue mats • Electric dehorners • Asking for help! Feeding Conditions • Feed Transport – Lifting feed bags, carrying bales of hay, location of feed bunks (individual feed areas or feed lot capacity) • Water Delivery – Carrying 5 gallon buckets (weight) – Diameter of handles (wire or plastic) • Weather Conditions • Topography/Terrain Feed Conditions Recommendations • • • • • • • • • Awareness of animal location and temperament Clear working areas and pathways Gloves and body cover Footwear with tread and ankle support Bucket handle grips Hose reels or automatic watering systems Utility vehicle Hay bale carrying stick Auger feeding systems Equipment and Maintenance • Fences and Gates • Ground Conditions • Equipment Repair • Bedding Stalls • Manure Disposal Equipment and Maintenance Recommendations • Button hook for threading leather or cord (tack repair) • Swivel stools or wheeled chairs • Door knob grips, alternative gate latches • Footwear quality • Manure spreaders instead of wheelbarrows • Pature Vac • Bedding options (weight/ease of removal) • Post hole diggers and drivers • Fence maintenance tools Training and Showing • Training • Fitting/Clipping/Grooming • Showing • Riding/Cow Work • Trailer Loading Training and Showing Recommendations • • • • • • Proper mounting/dismounting Portable steps or rope ladder, extendable stirrups Grooming chutes and/or stands Shoulder slings or straps for grooming equipment Proper training for hauling Balanced schedule for work Veterinary and Health Care • Occupational Hazards • Livestock Temperaments/Animals in Pain • Confined Spaces • Farrier Work Veterinary and Health Care Recommendations • Jar openers for medicines • Extendable grasp tools • Wheeled carts for equipment and tools • Livestock chutessqueeze chute • Magnetic belts and cuffs for tools Mobility Issues • Walking long distances over uneven ground • Driving for long periods of time (particularly with exposure to whole-body vibration) in tractors and utility vehicles • Horseback riding • Entering/exiting farm and ranch equipment • Obstructed pathways Mobility Recommendations • • • • • Automatic hitches Bump gates/cattle guards Car steering wheel covers for utility vehicles Footwear quality Proper mounting/dismounting of horses Buzz Word: Ergonomics • Universal Design of Assistive Technology? • Grip Diameter and Strength – rule of thumb • Angle of Joints • Weight-bearing Joints • Use proper body mechanics. – keeping your feet shoulder width apart – bending at the knees/hips- keeping the back straight – carrying the weight as close to the body as possible General Arthritis Management • Get a proper diagnosis- early detection • Get proper exercisewatch your weight and diet • Manage stress and pace yourself- work simplification • Be educated- unproven remedies Control of Arthritis Pain and Limitations • Personal Strength – Joint Strength and Body Mechanics – Bone and Muscle Strength • Medical/Alternative Options – – – – – Over-the-counter, topical ointments Supplements Hot/Cold Therapy Massage Injections, prescription medications Planning and Options • Balancing pain management and the desire/need to continue ranching/farming can be a very difficult decision. • Family Input/Responsibility and Sustainability • Farm Operation Modifications – Reducing herd size? – Farrow to finish or wean to finish? – Breeding horses, or training and selling? – What type of business planning is needed?
