Boodborne Pathogens and the Fire Service 29 CFR 1910.1030 By Jim McPadden 29 CFR 1910.1030 • Exposure Control Plan • Definition of an Exposure • Methods Of Compliance • PPE • Sharps • Engineering and Work Practice Controls 29 CFR 1910.1030 • Workplace Controls • Laundry and Waste Containment • Post Exposure and Evaluation • Housekeeping • Record keeping Exposure Control Plan • Employers must have a written plan • Designed to eliminate or minimize employee exposures • The plan must be accessible Must consist of the following • Exposure determination – List of jobs that potentially have occupational exposure – List of tasks and procedures in which occupational exposures occur – Exposure determination shall be made without regard to the use of PPE What is an Exposure? • A specific eye, mouth, other mucous membrane, non-intact skin, or parenteral contact with blood or other potentially infectious materials that results from the performance of an employee’s duties Methods of Compliance • Universal precautions shall be observed to prevent contact with blood or other potentially infectious materials • All body fluids shall be considered potentially infectious Personal Protective Equipment • Employer Shall provide, at no cost to the employee, PPE such as but not limited to– Gloves, gowns, face shields, eye protection, pocket masks, lab coats… Use of PPE • Employer shall ensure – PPE is used – PPE is appropriate size – Is readily accessible or issue to employees – PPE is appropriate for the task at hand PPE Contamination • All PPE that has been penetrated by potentially infectious material should be removed and/or replaced as soon as possible. • PPE should be place in appropriately designated containers • PPE should be disposed of or decontaminated Cleaning, Disposal, and Repair of PPE •Employer is responsible for all of the above at NO cost to the employee Engineering and Work Practice Controls • Employees shall provide – hand washing facilities which are readily accessible or – appropriate antiseptic hand cleaner in conjunction with towels – ensure that employees wash hands immediately after removal of gloves – ensure that employees wash hands and any other skin with soap and water, or – flush mucous membranes with water as soon a feasible after contact with potentially infected materials Handling of Sharps • Contaminated needles and other sharps shall not be bent or recapped • Unless a one-handed technique is used • Shearing or breaking needles is prohibited • Immediately after use sharps must be put into appropriate containers Sharps Containers • Labeled in accordance with 1910.1030 (g)(1)(i) • Puncture resistant • Color coded • Leak proof on side and bottom Sharps Injury Log • The employer shall maintain the log • Information recorded shall be confidential • Log info – Type and brand involved – Where the exposure occurred – How the event occurred Other Workplace Controls • Do not eat, drink smoke, apply cosmetics, or • handle contact lenses in work areas where there is reasonable likelihood of occupational exposure. All procedures involving potentially infectious materials should be done in such a manner as to minimize slashing, spraying, spattering, and generation of droplets of these substances. Contaminated Laundry • Handle as little as possible • Bag or contain where it is used • Transported in containers that are labeled in accordance with 1910.1030(g)(1)(i) Waste Containment • Placed in containers which are – Closable – Contain all contents – Prevent spillage or protrusion – Labeled in accordance with 1910.1030 (g)(1)(i) – Disposed of in accordance with applicable regulations Housekeeping The Employer shall• Ensure surfaces are disinfected after contact with potentially infectious material • Ensure that protective coverings are used when appropriate and replaced when contaminated Hepatitis B Vaccination • Made available by employer within 10 days – No cost to employee • Not mandatory – Employees declining vaccination need to sign waiver • If employee wants vaccination at a later date, the employer shall make it available Post-exposure Evaluation and Follow-up • Following a report of an exposure employers shall make available – an immediate confidential medical evaluation – You must document the route of exposure – Document the circumstances under which it occurred – Document the Identification of the source of the exposure Post-exposure Evaluation and Follow-up (continued) • The source individual’s blood shall be tested as soon as feasible • Results of the test shall be made known to the employee • Counseling shall be provided for employee Record Keeping • All blood test and result shall be kept confidential and secure • All documentation and medical records shall be kept by the employer for the duration of employment plus thirty years.
