You MUST register as a user
At your first login, you will select forgot
password link.
Choose: FORGOT PASSWORD
Type in your employee ID and select Reset
Password.
Login: YOUR CMS EMPLOYEE ID
•You will receive an email will come with the Access Code.
•Reset the password after entering Access Code.

your CMS email with a
hyperlink to the app in the
App Store
You “The User” will
download the app to your
iPhone / iPad
User opens the App
User Enters:
Faculty ID
Create Password
Faculty Email
Last Name

New User Sign Up Screen

Once Email verification is
successful you are ready to
set your passcode
Assigned School Name
The user will set up and
confirm a 4 digit pin for
security
After the Sign Up is completed
and a Pin has been created,
the user will only need to
enter that Pin to enter an
Incidents/behaviors into the
app
Submitting a Behavior Observation
To begin entering a Behavior
Observation Click on the
Behavior Link.
Assigned School Name
Once this screen is
complete, click on
Continue to advance to
the next screen
The Date will default to todays date. If you
wish to change it, tap on the field and select
a new date.
You will select the student name here. A list
will be provided. If you don’t know the
student name, you can save your report and
fill it in later. You will have 48 hours to
submit the report.
Select the Behavior Type here. There will be
4 categories to scroll through.
Verbal – Physical – Social – Computer

Images and/or video may be attached by
clicking here.
Once this screen is
complete, click on
Continue to advance to
the next screen.
Select the Place from a scrolling list.
By clicking in this field you will activate the
key pad. The next screen shows this field
activated.

Type in the details of the behavior.
Siri users will have the ability to speak the
behavior details by pressing this button.
Once this screen is
complete, click on
Continue to advance to
the next screen.
Enter any names of individuals involved. This
filed is not required to submit the report.
Once this screen is
complete, click on
Continue to advance to
the next screen.
Enter any names of individuals that may have
witnessed the behavior. This filed is not
required to submit the report.
The summary screen will give you a quick
glance at your report before you submit it.
Once the Behavior
Observation is
complete, click on Send
to submit the report.
Assigned School Name
If a report is not complete, it will show up in
your Pending link. You will have 48 hours
before the system forces the report through.
