Eligibility Process for Athletics A) How does my child become eligible for interscholastic sports? •He/She must receive an annual physical exam. •Parent submits completed medical and parent permission forms prior to the start of Pre-Season. B) Where do I find the required forms? •Crossroads Home Page displays the link to download the athletic forms. C) When do all the forms have to be submitted? •ALL forms are submitted to your school health office. Dates: Monday - July 9th – Nurses will begin to accept completed forms Wednesday - August 1st – Last day to submit forms to be eligible for the first day of Pre-Season D) Notifications for each sports season will be sent home approximately two weeks prior to the first day forms are accepted via hard copy, On-line Friday folder, and posted on the Crossroads website.
