Accreditation Requirements
Testing/calibration laboratory must fulfill these
requirement :
 has Quality Management System and technical
competence, which comply with SNI 19-17025-2000 or
ISO/IEC 17025-1999
 comply with the procedure and condition, which is
published by KAN
 has joined inter-laboratory comparison test which is
managed by KAN or other institution that has good
reputation in managing inter-laboratory comparison
program

Assessment is consist of two stage, that is :
 Sufficiency Audit
 Field Assessment
Assessment is conducted by two or more
assessor, that is :

Field Assessment
 Assessor will assess laboratory’s management
system and technical competence
 Assessor will give non-conformities report
which has agreed by both side
 Laboratory will make corrective action report at
least 3 months after assessment

 All assessment report will be investigated
by Technical Committee
 Investigation report will be reviewed in
KAN’s meeting.
 KAN’s meeting will decide whether to
accept the laboratory accreditation or not

ACCREDITATION PERIOD
Accreditation Certificate Validity is until 4
years
Six month before the certificate un-valid,
laboratory must be re-accredited by KAN

Surveillance
 KAN will survey the laboratory to make sure
that laboratory competence is still appropriate
with KAN accreditation criteria
 Surveillance is perform twice during the
accreditation validity

REVOCATION OF ACCREDITATION
Revocation of Laboratory Accreditation can
happen if :
 Laboratory fail in surveillance program
 There’s non-conformity in category 1
 Correction report can’t be done in time
 Laboratory has break the terms and
condition of KAN Accreditation criteria
 Laboratory still published certificate, scope
not accredited by KAN, with KAN’s logo on it
