Provide a safe temporary modified duty assignment for
employees who have been given temporary restrictions.
 Eligibility
• Employee who does not require time off but has temporary
restrictions which prevent them from performing their regular
position.
• Employee who has been off work for a period of time and is unable
to immediately perform their regular position.

Temporary Modified Duty Commitment
Lozier is committed to providing safe temporary modified duty assignments for employees who have been
given temporary restrictions by their treating physician. Such work will be provided on a temporary basis
subject to availability. Work will be assigned within the temporary restrictions provided by the treating
physician. Every effort will be made to place employees in a temporary modified duty assignment within
their own department and shift; but if necessary, employees will be placed wherever an appropriate
temporary modified duty assignment is available.
Due to your recent injury or illness your physician has provided temporary restrictions to your regular job
duties. Since you are unable to perform all the tasks of your regular position, Lozier is placing you in a
temporary modified duty assignment.
During this time, you must follow your restrictions at work and away from work. You are not to perform any
work for another employer or self-employment without notification to Lozier.
During this time you will continue to have appointments with your physician. Appointments will be
scheduled on your own time when at all possible.
Work Related Injury or Illness: If it is necessary to miss time from work for a work-related injury or
illness appointment, you must clock out before attending the appointment and clock in when you return
from the appointment.
Absences without a written statement from the treating physician will be
considered an unauthorized absence and you will not be paid for that time. Your supervisor will be notified
whenever an appointment is not kept. If appointments continue to be missed, a letter will be sent to you,
your supervisor, the plant manager and a copy will be placed in your personnel file.
Non-Work Related Injury or Illness: If it is necessary to miss time from work for a non-work related
injury or illness appointment, you must receive prior approval from plant management, similar to any other
absence from work.
At each appointment you will need to have a Patient Visit Form (Attachment A) or an equivalent form
completed and returned to the Leave Administrator the same day or no later than 24 hours after the
appointment. If restrictions change, your temporary modified duty assignment will be reevaluated.
This temporary modified duty assignment will be re-evaluated every 2 weeks or following each doctor’s
appointment. The program will not exceed 26 weeks unless the Company has granted an extension.

management process that is effective for the care of
overuse and strain/sprain symptoms.
 ART is a movement based soft tissue system that
targets scar-tissue to correct muscle function and
free nerve entrapments.
 By releasing scar tissue and correcting movement
and function, ART yields significant improvement
and/or resolution prior to conditions becoming
severe and costly (surgery, time-off work, disability).









 Lozier has contracted with a local ART provider
to provide this treatment option to Lozier
employees with work and non-work related softtissue symptoms:
 2 hours on site each week
 Conveniently located at the Health Clinic @
Lozier.
 Appointments coordinated through the
Omaha Safety Manager.
 Priority for appointments will be given to
those with work-related issues.
