Athletics
Students planning on turning out for sports need to have a
sports physical before participating in any sports practice.
Athletic Packets are available in the Main Office or on the
Centennial Website. Completed packets should be turned in
to the main office. Please make sure that the physician has
signed and completed his/her portion. Physicals are good
for 24 months from the date of doctor approval.
