(Your Name)
Step 1 (Applicant ID)
Enter a Certification ID or
SSN and click “Search”
Step 2 (Name Edit)
Enter/edit name and
address information
and click “Save
Changes”. If no
changes are needed,
click “Continue”.
Note: This date
and all other date
entries in
ExpressLane
require a
4-digit year.
Step 3 (Request)
Select the actions
you wish by checking
one or more boxes.
In this example, we
requested “Add a
Field by Approved
Program” and
“Upgrade to Level 6”
Scroll this page down to see
full Certification Status. Note
this teacher has Level 5
already, so “Upgrade to Level
5” was not listed as an option.
Step 3 (Confirmation)
Click “Proceed” unless
you need to “Start
Over” or “Quit”
Step 4
(Dialogue - Part 1)
Enter the required
information and click
“Proceed” unless you
need to “Back Up” or
“Quit”
The Dialogue is uniquely constructed to
fit your requests. In this case you
would need to enter the details of the
level 6 degree, which may include an
actual degree completion or an
alternative.
See the next slide for more dialogue ...
Step 4
(Dialogue - Part 2)
Select a field to be
added as the second
part of this request.
Click “Add”. You will
need to enter dates
indicating start of
employment in this
field.
The Dialogue will end after fields are
selected for addition. In this example,
we selected two requested actions upgrade and add a field by approved
program.
Repeat if there are
other fields to be
added with this
request.
Step 5
(Confirmation of All
Requests)
If the information is
correct, enter “yes”
and click on
“Finish”.
At this step, ExpressLane reminds you of the documents you will
need to support these requests.
Step 6
(Construction of the
FAX Cover Sheet)
Click to create the
FAX cover sheet,
then Return to the
ExpressLane Home
Page.
Step 7 (Print)
Use the FAX Cover
Sheet within 30 days.
Use the checklist to
make sure all required
documents are
included. FAX them all
to the PSC.
It is critical that you fill
in this blank accurately
and legibly.

If the applicant answered Yes to any
ethics question, shade in this circle.
Ready for More …
Listings of your
prior requests will
appear on the
ExpressLane startup
page
Track your case status
with these three
indicators.
If you lose your first
cover sheet, you may
print another one.
Warning: after you
print a second cover
sheet, the first one is
no longer valid; the first
cover sheet won’t
“work” if you send it in
after replacement.
If you change your
mind and you want to
“recall” this request,
you may do so with the
“withdraw” option. This
must be done quickly.
The withdraw option
will not be available
after the case has been
assigned to an
evaluator for work.
Listings of your prior
requests will appear on
the ExpressLane startup
page. Pick a category
and click “Refresh”.
Track your case status
with this column
If you lose your first
cover sheet, you may
print another one.
If you change your mind you
may “withdraw” an ExpressLane
request. But this must be done
before we receive your fax! The
withdraw option will not be
available after the fax has been
checked-in and a case has been
assigned to an evaluator for
work.
Some Helpful Hints ...
It helps the evaluator
when important items
are circled. Do not
highlight with a pen of
any color. This is
especially true for
transcripts. Do not
highlight on transcripts.
However it is helpful to
circle degree award
information and GPA on
transcripts.
If a required document
is already on file at the
PSC, a note to that
effect is very helpful,
too.
We may need to call your office
if the FAX transmission was bad,
so if you prefer another number,
write it above this one.
This number is important. If you
don’t tell us how many pages to
expect, we won’t know if the
transmission was complete.
If the applicant marked “Yes” to any
ethics affirmation questions, fill in
this circle to alert PSC staff.
