collaboratively to better measure care
outcomes:




Revise standards and measures for all settings and
services to more accurately measure quality outcomes
Use HAB evaluation questions to guide us in evaluating
our program
Continue to work with our Title III program and the AETC
to effect process changes that will improve quality of care
Incorporate the six IOM aims: safe, effective, patientcentered, timely, efficient, and equitable
