• First name first, last name last, only first letter of
each name capitalized, in paragraph format in a
Word document
• Example –

Grade 1 – Jane Doe, John Smith, Ringo Star, Teddy
Bear, Yogi Bear, Hannah Montana, Suzie Que, etc.

• DO Identify the grade level and school.
• DO Send before mid-term of the next
quarter
• Worst Way! Do NOT send this way…
– Excel format, last name first, first name last,
all CAPS, separate cells in a Word table, etc.
