Overview
A printable LTO Evaluation Form is available on our
website (www.dotbu.org.)
LTO Evaluation Process
 LTO position must be at least four (4) months long.
 Principals should meet with the OT prior to the evaluation to
provide an overview of the process and areas of consideration,
i.e. classroom management, assessment and evaluation,
planning, etc.
 Observation/Evaluation date should be pre-determined by
both parties.
 Evaluation process should promote professional dialogue
where effective feedback is provided.
