Project Aim and Goals
Aim: Increase the opportunity to provide
donor organ & tissue to patients
needing transplants by….
• Increasing the timeliness of notification phone
calls to Gift Of Hope (the regional organ
procurement agency). Timeliness is defined as
within 2 hours of identification of potential donor
status
• Increasing the organ/tissue donation conversion
rate to 75%

• Revised policy for Organ/Tissue donation

Staff reminders to call in ALL deaths within
2 hours of expiration
Continue to alert Gift of Hope to imminent
deaths, especially prior to any terminal
extubation. Gift of Hope comes out after
notification of death.
Benchmark with other transplant centers
and identify strategies for improvements
