    Reception completes intake triage: photocopies id/income tax/bank statements/medicals and/or residency Intake worker case manages clients aged 45-65+ years Intake worker case manages clients aged 30-44 years Intake worker case manages clients aged 18-29 years  Case workers interview applicants one on one  Case workers present application to Director for review and decision. If approved, client receives basic needs for the month after 3pm that same day. If no decision is made, file remains in pending until all documentation is verified 4

       ALL clients must complete the financial needs assessment before an application is approved 2 pieces of identification on file; one with photo and other with local mailing address Income tax or Notice of Assessment from CRA for latest tax year (e.g. 2015-16 fiscal requires 2014 NOA) Or 60 days most current bank statement This ensures client has a financial need; no financial claw back from AANDC will occur for benefits paid Emergency foods will be issued until applicant submits required documentation to social department Home visits conducted to verify residency 5

   Past compliance ratios resulted in financial claw backs of thousands of dollars for SFN Social department was informed in December 2014 that auditors would be here 2 weeks in January 2015 Approximately 278 files were pulled for review      2009-2010 - $188,438.62

2010-2011 - $161,755.69

2011-2012 - $185,412.98

2012-2013 – Waiting for compliance results; auditors were here in January 2015 EFFECTIVE 2012/2013, the First Nation is funded at 100% ratio 6
