To ensure effective working the Integrated Services
Group will review and monitor all cases that are being
dealt with by ISCAN
In order to evaluate how ISCAN is working, we would
like to involve parents who will receive the service so
that we can obtain your views on what you think about
the service. A number of parents/carers will be asked to
be part of the service evaluation.

What is ISCAN?
What Happens Next?
It is an Integrated Service where professionals from
Health, Education and Social Care work together to
provide a service for families of children with
additional needs.For the age range 0-5 years.
Step One:
ISCAN will provide an effective and trusted service
for families and improve partnership working.
At this meeting a “Lead Professional” will be
allocated to your case. An assessment of your
child’s and family needs will be undertaken.
How will the Service work?
Step Two:
•A family will be referred to the Service, this will
only be done with your consent..
A Team Around the Child (TAC) Meeting will be
arranged by the Lead Professional
•Caerphilly Children’s Centre will receive and
process the referrals.
Step Three:
•Families involved with ISCAN will be contacted
by the Information & Advice Officer within 2
working days of receiving a referral. You will
receive information about the service and process.
You will be asked to provide information about
your child’s and family needs
•Share information
The ISCAN Multi-Agency Meeting will meet on a
weekly basis to discuss referrals received.
The Team Around the Child (TAC) Meeting will:
•Jointly agree possible solutions and appropriate
actions
•Complete an action plan and agree next steps
