• • Use “ID Lookup” button or link on Login Page – see screen instructions Provide the correct personal information (SSN, DOB or Name) to get your User ID (starts with ‘910’) – How do I get my PIN?

• • For first-time users, the PIN is your birthdate in MMDDYY format – see screen instructions You will be prompted to reset it when you first log in to the Self-Service system (any 6 digits/numbers) – What if I forgot my PIN?

• • On Login Page, enter User ID and click “Forgot PIN” button – see screen instructions Answer the Security Question correctly and then reset your PIN – What if my PIN is disabled?
