Two cycles in both ST1 (clinicians only) and ST3 (clinicians and nonclinicians),
with the option of repeating in ST2 if unsatisfactory in ST1
 Patient Satisfaction Questionnaire (PSQ)
One cycle in ST1 or ST2, when in primary care, and one in ST3
