Naming Networked/Affiliate Name as new broker
Effective Date of Change (Renewal Date)
Must be on Insured’s Letterhead & signed by authorized person
Completed Application is required
Check with your underwriter regarding timing of BOR’s by
carrier
As a rule, our carriers will not accept mid-term BOR’s
