• Raise the Voice is a program funded by the
Robert Wood Johnson Foundation and created
by the American Academy of Nursing (AAN) to
highlight how nurses can help transform
America’s health care system
• Nurses have the solutions to turn our nation’s
expensive, fragmented and inaccessible health
care system into one that is safe, effective,
affordable and personalized.
• Raise the Voice has documented powerful
examples of research-based solutions
developed by Nurse Edge Runners that tackle
serious health care challenges such as the
uninsured, rising costs of care and health
disparities
• If adopted nationally, these programs could
increase the efficiency of the health care system
while reducing costs
• Some examples:
– Evercare – A program developed by two nurse
practitioners that serves people who have long-term
or advanced illness, are older or have disabilities.
The program overcame fragmentation in the health
care system, reduced emergency treatments and
hospitalizations and lowered costs to Medicare and
Medicaid. It was so successful it has been adopted in
35 states and is now serving more than 120,000
people.
– Nurse-Family Partnership – A home visitation program that
provides care via a registered nurse to first-time mothers for the
first two years of a child’s life. The program improves pregnancy
outcomes, child health and development, and the economic selfsufficiency of the family. It returns an estimated $2.88 per dollar
invested and has been adopted in 23 states nationwide.
– APN Transitional Care Model – A transitional care program led
by advanced practice nurses (APNs) working in conjunction with
a patient’s entire health care team to ensure a smooth transition
from hospital to home. The program improves post-discharge
outcomes, lowering rates of re-hospitalization and thereby
reducing costs. A four-year trial resulted in a savings of
approximately $5,000 per Medicare patient.
