• Filled out 2 times a day
– AM (before lunch) and PM (after lunch) performance
• All students start at +3 Ready to Learn
• Points earned are recorded by student on
behavior chart and brought home daily for parent
review (In child’s HW folder)
• Please take a moment each night to honor your
child for their good behavior or discuss any
needed improvements, this behavior log is
informational and not intended to be punitive.

(reinforces vertical and horizontal addition facts)
*Maximum points a student could earned per day is $0.10
*Maximum points a student could earned per week is $0.50
