qualifying organizations for greater impact and efficiency
HSC Analysis = New Strategy developed with data review and
analysis and support from other County Departments
Development of 3 Tier model*: Mini Grants (agency capacity
building), Basic Services (maintenance & stability), Best
Practices (proven programs)
*Based on the overall disposition of grant applications received, determination of needs
and availability of funding, the Human Services Commission may make
recommendations to the Board of Supervisors outside of the guidelines that follow.

Funding Guidelines Cont.
 Mini Grants – one year commitment, may re-apply
annually
 Basic Services and Best Practices Grants –
 Utilize HUD guidelines on income, race and ethnicity
 Document upload requirements in ZG
 Describe how you will reach the target population and
how programs are culturally competent and accessible
for all persons needing the services including nonEnglish speaking populations and those without transit
 Basic Services grants may be renewed for 2016-17 and
2017-18 depending upon performance
 Best Practices Grants are (3) three years
Tier One – One Year Mini Grants
Purpose: Catalysts for Capacity Building
($5,000 – 7,000 with $50,000 total)
Examples: organizational development, systems change,
agency capacity building (i.e., training including staff and
boards of directors, collaboration/data sharing; research
(i.e., to develop a new program, extend service reach,
conduct asset mapping, conduct a media /marketing /
advocacy campaign, develop strategy on how to move
toward evidence-based best practice)

Tier Two – Basic Services
(1-3 years)
Purpose: Safety net programs, maintenance,
prevention ($20,000-$50,000 with $500,000 total)
• Priorities: food, shelter, transportation, services
which provide freedom from abuse (i.e., spousal,
seniors, children through violence, physical,
sexual and emotional assault)
• Other services that support families, seniors,
youth, disabled, homeless populations
• Reporting and Invoicing - quarterly
• Monitoring; onsite visits as needed

Tier Three – Best Practices
(3 years)
Purpose: ($50,000-$100,000 with $450,00 total)
The overarching purpose is to achieve measurable results in
assisting individuals and families in need crossing all
socioeconomic boundaries. Combines the best research with
agency expertise and client needs, and where:
 Results are clearly articulated, data driven, research




supported, derived from experience, peer reviewed,
collaborative, scalable and adaptable
A proven model is either demonstrated by an applicant or tied
to industry standards from reputable institution or
government agency
At the end of the three-year grant cycle clients life
circumstances have clearly improved
More cost effective and efficient
Evaluation and monitoring are more extensive
Definition of Evidence
Combines the best research with clinical expertise and client needs
(i.e., location, culture, health, language). Includes:
• Contextual evidence
– Measurable community factors (data, assessment, focus
groups)
– What is acceptable, feasible and useful in local setting
• Research evidence
– Inquiry, evaluation, design and implementation
– Observable, replicable, credible, verifiable
• Experiential evidence
– Insight, understanding, skills, expertise gained over time
– Knowledge of community through real world experience
• Adapt Model:
Assess target population, identify components of proven model,
modify model, document modifications, pilot the modified model
Note: If your agency has been using an evidence based system that has proven
effective or have one that is emerging you are encouraged to submit an
application for review

 Describe proposed program (new/ongoing), agency

mission, years in operation, staff capacity and expertise,
volunteer skills and level of commitment
Demonstrate that the proposed program is backed by
clearly articulated proven results that can measure success
(i.e., from data, evaluations, tracking clients, conducting
pre/post assessments, surveys, focus groups)
At the end of the 3 year cycle a clients life
circumstances will have clearly improved
Constituency involved (program; board members)
Location, target population and units of service are clearly
identified; does the program differ from similar programs?
Describe how “reach” and “access” are achieved
Minimum quarterly invoicing and reporting and site visits
as needed, minimum annually
Utilize HUD’s income, race and ethnicity guidelines
