Born in a promise between two sisters -- Nancy Brinker to her sister, Susan G. Komen, who died from breast cancer at the age of 36 – our vision to see a world without breast cancer has become the vision of millions.

We are more than 100,000 survivors and local activists in more than 120 cities and communities, mobilizing more than 1 million friends and neighbors every year through events like the Komen Race for the Cure® Series—the world's largest and most successful awareness and fundraising event for breast cancer.
We are advocates at the local, state and federal level, fighting for the screening and treatment programs that save lives and the research that brings us closer to the cures.
We are global citizens working with local health groups around the world and through our Web site, komen.org, to help millions of women in nearly 200 countries overcome the social, cultural and economic barriers to breast health and treatment.

Breast Cancer in the United States 2009 Estimate

2009 Breast Cancer Fast Facts
One woman is diagnosed with breast cancer every three minutes and one will die of breast cancer every 13 minutes in the US.
Breast Cancer is second only to lung in cancer deaths among women in the US
Except for skin cancers, breast cancer is the second most frequently diagnosed cancer among women in US
In the US, white women age 40 and older were more likely to report having a mammogram in the past two years, than any other racial or ethnic group.
In the US, the lowest prevalence of mammography screening in the past two years occurred among women who do not have health insurance, followed by immigrant women.

Grantees are asked to attend/support at least 1 Komen Event through the grant period.
Grantees are asked to display their granted program materials in the Grantee Booth at Race for the Cure ®, March 12, 2011.

Who is Eligible to Apply?
Applicants must ensure any past and current Komen-funded grants or awards are up-to-date and in good standing.
Institutions must be US non-profit (federally tax-exempt) organization, e.g. nonprofit organizations, educational institutions, government agencies and Native Americans are eligible. Applicant organizations must be located in or providing services in Charlotte, Collier, Glades, Hendry and/or Lee Counties.
Program must be specific to breast health and or breast cancer; e.g. if a program is a combined breast and cervical cancer program, funding may only be requested for the breast cancer portion of the program.

2011-12 Funding Priorities
Increasing breast health awareness and education activities.
Increasing resources available to the uninsured/underinsured, low income population.
Reducing/removing barriers such as transportation, child care, and scheduling issues for screening/treatments.
Improving and increasing resources for diagnostic testing, staging procedures, and treatment for uninsured/underinsured, low income population.
Increasing efforts to educate and support newly diagnosed patients and breast cancer survivors about local programs/services available to help with quality of life issues.

Screening/Treatment Grants
- The Southwest Florida Affiliate of Susan G. Komen for the Cure® is offering grants up to $200,000 for programs that perform screening mammograms, diagnostics and/or breast cancer treatment services for uninsured or underinsured individuals.
Education/Support Grants -
The Southwest Florida Affiliate of Susan G. Komen for the Cure® is offering grants up to $100,000 for programs that provide breast cancer/breast health education and/or support services that target the medically uninsured and underserved populations. Education programs should be culturally appropriate for targeted populations and
must
link program participants with breast cancer screening and follow-up, as necessary. Support programs include emotional support programs and temporary gap funding for breast cancer survivors and their caregivers.

Categories for 2011/12
Breast Health Navigator (BHN) Grants -
The Southwest Florida Affiliate of Susan G. Komen for the Cure® through its previous grantees has experienced the impact Breast Health Navigators make in the life of individuals dealing with breast cancer. Therefore the Affiliate wants to foster the development of new BHN programs in areas of need. New Breast Health Navigator programs will require a job description and salary requests may be made for up to 100% the first year, up to 75% the second year and up to 50% the third and future years. As with other grants the applicant will need to show sustainability of the program.
Personnel Expense
- Applications requesting personnel expense, a cap of up to 50% of an individual’s salary can be included in the grant request and only the portion of the individual’s salary that pertains to the grant program may be included in the grant application. For example, if the total salary is $50,000 and the individual spends 50% of their time on the grant program, then only 50% of the salary ($25,000) may be included in the request. A complete job description must be included in the grant request.

Funds May be Used For:
Clinical services or breast care costs.
Direct program supplies, e.g. breast models, flip charts, videos, that further the Komen educational mission.
Other Expenses - Disposable supplies related to the program, e.g. copy paper, pens, etc., not to exceed 5% of total grant request. The budget must include category-level detail. For example, if the program costs include printing flyers to promote the program (250 flyers = $25.00).

If Awarded:
1. Grant Contract legal binding agreement and will outline the following: • • • Grant Payments Program reports will be required as identified in the Grant Contract. Grantees will be expected to participate in at least one (1) Komen event (volunteering) during the grant period. • Grantees are expected to display program materials at the Race for the Cure Grantee Booth March, 2012.
• Grantees will be required to be a resource in compiling the Community Profile during the Grant period.
• • Grantees are required to follow Komen Education Messages Insurance Requirement for programs providing patient care 2. Site visits will be performed by the Grant Committee during the year.

Insurance Requirement
Grant contracts include new verbiage this year concerning insurance and indemnity:
Grantee agrees to maintain the following insurance during the term of this Grant Contract: (i) commercial general liability insurance with combined limits of not less than $1,000,000 per occurrence and $2,000,000 in the aggregate for bodily injury, including death, and property damage; and (ii) excess/umbrella insurance, excess to the insurance set forth in (i) above, with a limit of not less than $5,000,000. Grantee shall name Komen Affiliate as an additional insured under its commercial general liability insurance policy solely with respect to the Breast Cancer Program and any additional policies and riders entered into by Grantee in connection with the Breast Cancer Program.

If Awarded a Komen Grant: Susan G. Komen for the Cure® is a resource of information about breast cancer and breast health for people all over the world. To reduce confusion and reinforce learning, we
require
that grantees provide up to date educational messages and materials that are consistent with those promoted by Komen for the Cure®.
