INDICAZIONI PER UNA CORRETTA COMPILAZIONE IN FORMATO ELETTRONICO
Il nuovo listino materiali di riferimento e ring test del Laboratorio Standard Latte per
l’anno 2017, si presenta composto da un totale di 5 pagine.
Le prime tre pagine rappresentano la parte contrattuale tra il Cliente ed il LSL.
Ciascuna di queste pagine deve essere datata, firmata e timbrata.
Le pagine 4 e 5 riportano una descrizione dettagliata ed i corrispondenti codici dei
prodotti forniti dal Laboratorio Standard Latte.
ISTRUZIONI
1) Compilare tutti i campi previsti alla pagina 1. Stampare, datare, firmare e
timbrare.
2) Indicare, alla pagina 2, la data prescelta per l’invio dei materiali di riferimento
e/o dei ring test, ed il numero di serie che si desidera ricevere (es.: indicando
3 sul prodotto FT001 del 10 gennaio verranno fornite 3 serie di quel prodotto,
ovvero 30 provette in totale). Stampare, datare, firmare e timbrare.
a. Nel caso di richiesta del certificato con Solidi Totali (FT/QS 812) per
specie diverse da quella bovina, si prega di specificare nello spazio
“Note” previsto alla pagina 3, la data e la specie.
3) Specificare, alla pagina 3, la modalità di spedizione prescelta. Stampare,
datare, firmare e timbrare.
4) Utilizzare lo spazio “Note”, presente in pagina 3, per indicare specifiche
condizioni.
5) Inviare il contratto, così compilato, per mezzo mail ([email protected] o
[email protected] ) o fax (+39 066678811).

INSTRUCTIONS TO FILL THE CONTRACT
The new price list for 2017 of the Reference Materials and Proficiency Tests, is
condensed in 5 pages in total.
The first three pages represent the legal contract form between Customer and LSL.
Each of these pages has to be dated, stamped, and signed.
Pages 4 and 5 describe in detail the products and the correspondence codes
produced by Laboratorio Standard Latte.
DIRECTIONS
1) Fill correctly all the fields required on page 1. Print, date, stamp and sign.
2) Tick, on page 2, the date of shipment and specify the number of series
desired. Print, date, stamp and sign.
a. In the case the customer desires a certificate comprehensive of Total
Solids (FT812) for species different than cow, please, specify the date
and the species in the box “Notes” included on page 3.
3) Specify, on page 3, the shipping conditions. Print, date, stamp and sign.
4) For specific and more detailed requests, please, use the box “Notes” on page
3.
