1. Complete the SMART Goals worksheet. Write a goal specific to the 2nd half of
the term.
2. In a Google doc, state your SMART goal for the remainder of the term. Include
an evaluation of your SMART goal using the 4 quadrants of the Decisional
Balance worksheet.
3. Mid-Term Progress Report Due Friday, May 6th. Must be signed by all CMC
and CCC instructors. Worth 25 points.
