Performance Evaluation
Annual performance reviews are a key
component of employee development.
 The performance review is intended to
be a fair and balanced assessment of an
employee’s performance.
 Employee sets goals and evaluates him- or
her- self on these goals, as well as basic
job performance. This is then discussed
with employee’s supervisor.


For each category below, rate your performance in
this course, this semester using the E, M, D scale:
1. Dependability & Cooperation (punctual, reliable, and
works well with other students and teacher)
2. Quality of Work (accuracy, thoroughness, and
eagerness to improve)
3. Work Performance (ability to do a majority of work
above average; works well under pressure)
4. Professionalism (demonstrates self-control; makes
sound decisions)
5. Attitude Towards Work (courteous, interested; willing
to work at difficult tasks; takes and follows directions
cheerfully)
6. Motivation/Initiative (motivated to complete work on
time and properly; takes initiative to start new tasks,
offer ideas, and finish work creatively)

you must complete as part of your job as
